This commit standardizes the output format for LSNs to ensure consistent
representation across various tools and messages. Previously, LSNs were
inconsistently printed as `%X/%X` in some contexts, while others used
zero-padding. This often led to confusion when comparing.
To address this, the LSN format is now uniformly set to `%X/%08X`,
ensuring the lower 32-bit part is always zero-padded to eight
hexadecimal digits.

This gets rid of the bespoken ProcessWalRcvInterrupts() function,
which lets walreceiver terminate at any CHECK_FOR_INTERRUPTS() call.
And it's less code anyway.
We can now use the standard libpqsrv_connect_params() libpq wrapper
from libpq-be-fe-helpers.h, removing more code. We attempted to do
that earlier already in commit 728f86fec6, but that was reverted
because it didn't call ProcessWalRcvInterrupts() and therefore didn't
react to shutdown requests. Now that ProcessWalRcvInterrupts() is
gone, it works. As stated in that commit, this also leads to
libpqwalreceiver reserving file descriptors for libpq conncetions,
which is nice.

This commit adds two improvements related to the monitoring of WAL
writes for the WAL receiver.
First, write counts and timings are now counted in pg_stat_io for the
WAL receiver. These have been discarded from pg_stat_wal in
ff99918c62 due to performance concerns, related to the fact that we
still relied on an on-disk file for the stats back then, even with
track_wal_io_timing to avoid the overhead of the timestamp calculations.
This implementation is simpler than the original proposal as it is
possible to rely on the APIs of pgstat_io.c to do the job. Like the
fsync and read data, track_wal_io_timing needs to be enabled to track
the timings.
Second, a wait event is added around the pg_pwrite() call in charge of
the writes, using the exiting WAIT_EVENT_WAL_WRITE. This is useful as
the WAL receiver data is tracked in pg_stat_activity.

The WAL receiver and WAL summarizer processes gain each one a call to
pgstat_report_wal(), to make sure that they report their WAL statistics
to pgstats, gathering data for pg_stat_io.
In the WAL receiver, the stats reports are timed with status updates sent
to the primary, that depend on wal_receiver_status_interval and
wal_receiver_timeout. This is a conservative choice, but perhaps we
could be more aggressive with the frequency of the stats reports. An
interesting historical fact is that the WAL receiver does writes and
syncs of WAL, but it has never reported its statistics to pgstats in
pg_stat_wal.
In the WAL summarizer, the stats reports are done each time the process
waits for WAL.
While on it, pg_stat_io is adjusted so as these two processes do not
report any rows when IOObject is not WAL, making the view easier to use
with less rows.
Two tests are added in TAP, checking statistics for the WAL summarizer
and the WAL receiver. Status updates in the WAL receiver are currently
possible in the recovery test 001_stream_rep.pl.

Introduce new postmaster_child_launch() function that deals with the
differences in EXEC_BACKEND mode.
Refactor the mechanism of passing information from the parent to child
process. Instead of using different command-line arguments when
launching the child process in EXEC_BACKEND mode, pass a
variable-length blob of startup data along with all the global
variables. The contents of that blob depend on the kind of child
process being launched. In !EXEC_BACKEND mode, we use the same blob,
but it's simply inherited from the parent to child process.

While IWYU also suggests to *add* a bunch of #include's (which is its
main purpose), this patch does not do that. In some cases, a more
specific #include replaces another less specific one.
Some manual adjustments of the automatic result:
- IWYU currently doesn't know about includes that provide global
variable declarations (like -Wmissing-variable-declarations), so
those includes are being kept manually.
- All includes for port(ability) headers are being kept for now, to
play it safe.
- No changes of catalog/pg_foo.h to catalog/pg_foo_d.h, to keep the
patch from exploding in size.
Note that this patch touches just *.c files, so nothing declared in
header files changes in hidden ways.
As a small example, in src/backend/access/transam/rmgr.c, some IWYU
pragma annotations are added to handle a special case there.

This patch provides support for regular (non-replication) connections in
libpqrcv_connect(). This can be used to execute SQL statements on the
primary server without starting a walsender.
A new API libpqrcv_get_dbname_from_conninfo() is also added to extract the
database name from the given connection-info.
Note that this patch doesn't change any existing functionality but later
patches implementing the slot synchronization will use this functionality
to connect to the primary server to fetch required slot information.

This commit implements a new replication command called
ALTER_REPLICATION_SLOT and a corresponding walreceiver API function named
walrcv_alter_slot. Additionally, the CREATE_REPLICATION_SLOT command has
been extended to support the failover option.
These new additions allow the modification of the failover property of a
replication slot on the publisher. A subsequent commit will make use of
these commands in subscription commands and will add the tests as well to
cover the functionality added/changed by this commit.

For clarity, have separate functions for *creating* the shared memory
and semaphores at postmaster or single-user backend startup, and
for *attaching* to existing shared memory structures in EXEC_BACKEND
case. CreateSharedMemoryAndSemaphores() is now called only at
postmaster startup, and a new AttachSharedMemoryStructs() function is
called at backend startup in EXEC_BACKEND mode.

f0efa5aec introduced the concept of "read-only" StringInfos which makes
use of an existing, possibly not NUL terminated, buffer.
Here we adjust two places that make use of StringInfos to receive data
to avoid using appendBinaryStringInfo() in cases where a NUL termination
character is not required. This saves a possible palloc() and saves
having to needlessly memcpy() from one buffer to another.
Here we adjust two places which were using appendBinaryStringInfo().
Neither of these cases seem particularly performance-critical. In the
case of XLogWalRcvProcessMsg(), the appendBinaryStringInfo() was only
appending 24 bytes. The change made here does mean that we can get rid
of the incoming_message global variable and make that local instead.
The apply_spooled_messages() case applies in logical decoding when
applying (possibly large) changes which have been serialized to a file.

Since C99, there can be a trailing comma after the last value in an
enum definition. A lot of new code has been introducing this style on
the fly. Some new patches are now taking an inconsistent approach to
this. Some add the last comma on the fly if they add a new last
value, some are trying to preserve the existing style in each place,
some are even dropping the last comma if there was one. We could
nudge this all in a consistent direction if we just add the trailing
commas everywhere once.
I omitted a few places where there was a fixed "last" value that will
always stay last. I also skipped the header files of libpq and ecpg,
in case people want to use those with older compilers. There were
also a small number of cases where the enum type wasn't used anywhere
(but the enum values were), which ended up confusing pgindent a bit,
so I left those alone.

Physical walsenders can't send data until it's been flushed; logical
walsenders can't decode and send data until it's been applied. On the
standby, the WAL is flushed first, which will only wake up physical
walsenders; and then applied, which will only wake up logical
walsenders.
Previously, all walsenders were awakened when the WAL was flushed. That
was fine for logical walsenders on the primary; but on the standby the
flushed WAL would have been not applied yet, so logical walsenders were
awakened too early.

This role can be granted to non-superusers to allow them to issue
CREATE SUBSCRIPTION. The non-superuser must additionally have CREATE
permissions on the database in which the subscription is to be
created.
Most forms of ALTER SUBSCRIPTION, including ALTER SUBSCRIPTION .. SKIP,
now require only that the role performing the operation own the
subscription, or inherit the privileges of the owner. However, to
use ALTER SUBSCRIPTION ... RENAME or ALTER SUBSCRIPTION ... OWNER TO,
you also need CREATE permission on the database. This is similar to
what we do for schemas. To change the owner of a schema, you must also
have permission to SET ROLE to the new owner, similar to what we do
for other object types.
Non-superusers are required to specify a password for authentication
and the remote side must use the password, similar to what is required
for postgres_fdw and dblink. A superuser who wants a non-superuser to
own a subscription that does not rely on password authentication may
set the new password_required=false property on that subscription. A
non-superuser may not set password_required=false and may not modify a
subscription that already has password_required=false.
This new password_required subscription property works much like the
eponymous postgres_fdw property. In both cases, the actual semantics
are that a password is not required if either (1) the property is set
to false or (2) the relevant user is the superuser.

In the 90s we needed to deal with computers that still had the
pre-standard signal masking APIs. That hasn't been relevant for a very
long time on Unix systems, and c94ae9d8 got rid of a remaining
dependency in our Windows porting code. PG_SETMASK didn't expose
save/restore functionality, so we'd already started using sigprocmask()
directly in places, creating the visual distraction of having two ways
to spell it. It's not part of the API that extensions are expected to
be using (but if they are, the change will be trivial). It seems like a
good time to drop the old macro and just call the standard POSIX
function.

We'd like to use TimestampDifferenceMilliseconds with the stop_time
possibly being TIMESTAMP_INFINITY, but up to now it's disclaimed
responsibility for overflow cases. Define it to clamp its output to
the range [0, INT_MAX], handling overflow correctly. (INT_MAX rather
than LONG_MAX seems appropriate, because the function is already
described as being intended for calculating wait times for WaitLatch
et al, and that infrastructure only handles waits up to INT_MAX.
Also, this choice gets rid of cross-platform behavioral differences.)
Having done that, we can replace some ad-hoc code in walreceiver.c
with a simple call to TimestampDifferenceMilliseconds.
While at it, fix some buglets in existing callers of
TimestampDifferenceMilliseconds: basebackup_copy.c had not read the
memo about TimestampDifferenceMilliseconds never returning a negative
value, and postmaster.c had not read the memo about Min() and Max()
being macros with multiple-evaluation hazards. Neither of these
quite seem worth back-patching.

Avoid having walreceiver code know explicitly about the precision
and underlying datatype of TimestampTz. (There is still one
calculation that knows that, which should be replaced with use of
TimestampDifferenceMilliseconds; but we need to figure out what to do
about overflow cases first.)
In support of this, provide a TimestampTzPlusSeconds macro, as well
as TIMESTAMP_INFINITY and TIMESTAMP_MINUS_INFINITY macros. (We could
have used the existing DT_NOEND and DT_NOBEGIN symbols, but I judged
those too opaque and confusing.)
Move GetCurrentTimestamp calls so that it's more obvious that we
are not using stale values of "now" anyplace. This doesn't result
in net more calls, and might indeed make for net fewer.
Avoid having a dummy value in the WalRcvWakeupReason enum, so that
we can hope for the compiler to catch overlooked switch cases.

Commit 05a7be93 changed the timing of the first reply sent by a
walreceiver, which caused a few TAP tests that call wait_for_catchup()
when they haven't actually streamed anything yet to wait ~10 seconds
(wal_receiver_status_interval).
Before commit 05a7be93 the initial reply was sent after 100ms, but
there's no reason not to send it immediately as a slight improvement.
Do the same for HS feedback for consistency.

Commits cf112c12 and a0dc8271 were a little too hasty in getting rid of
the pg_ prefixes where we use pread(), pwrite() and vectored variants.
We dropped support for ancient Unixes where we needed to use lseek() to
implement replacements for those, but it turns out that Windows also
changes the current position even when you pass in an offset to
ReadFile() and WriteFile() if the file handle is synchronous, despite
its documentation saying otherwise.
Switching to asynchronous file handles would fix that, but have other
complications. For now let's just put back the pg_ prefix and add some
comments to highlight the non-standard side-effect, which we can now
describe as Windows-only.

pread() and pwrite() are in SUSv2, and all targeted Unix systems have
them.
Previously, we defined pg_pread and pg_pwrite to emulate these function
with lseek() on old Unixen. The names with a pg_ prefix were a reminder
of a portability hazard: they might change the current file position.
That hazard is gone, so we can drop the prefixes.
Since the remaining replacement code is Windows-only, move it into
src/port/win32p{read,write}.c, and move the declarations into
src/include/port/win32_port.h.
No need for vestigial HAVE_PREAD, HAVE_PWRITE macros as they were only
used for declarations in port.h which have now moved into win32_port.h.

MemSet() with a value other than 0 just falls back to memset(), so the
indirection is unnecessary if the value is constant and not 0. Since
there is some interest in getting rid of MemSet(), this gets some easy
cases out of the way. (There are a few MemSet() calls that I didn't
change to maintain the consistency with their surrounding code.)

Generally if a role is granted membership to another role with NOINHERIT
they must use SET ROLE to access the privileges of that role, however
with predefined roles the membership and privilege is conflated. Fix that
by replacing is_member_of_role with has_privs_for_role for predefined
roles. Patch does not remove is_member_of_role from acl.h, but it does
add a warning not to use that function for privilege checking. Not
backpatched based on hackers list discussion.

This moves the functions related to performing WAL recovery into the new
xlogrecovery.c source file, leaving xlog.c responsible for maintaining
the WAL buffers, coordinating the startup and switch from recovery to

All such code deals with this global variable in one of three ways.
Sometimes the same functions use it in more than one of these ways
at the same time.
First, sometimes it's an implicit argument to one or more functions
being called in xlog.c or elsewhere, and must be set to the
appropriate value before calling those functions lest they
misbehave. In those cases, it is now passed as an explicit argument
instead.
Second, sometimes it's used to obtain the current timeline after
the end of recovery, i.e. the timeline to which WAL is being
written and flushed. Such code now calls GetWALInsertionTimeLine()
or relies on the new out parameter added to GetFlushRecPtr().
Third, sometimes it's used during recovery to store the current
replay timeline. That can change, so such code must generally
update the value before each use. It can still do that, but must
now use a local variable instead.
The net effect of these changes is to reduce by a fair amount the
amount of code that is directly accessing this global variable.
That's good, because history has shown that we don't always think
clearly about which timeline ID it's supposed to contain at any
given point in time, or indeed, whether it has been or needs to
be initialized at any given point in the code.

Previously, walreceiver always closed the currently-opened WAL segment
and created its archive notification file, after it finished writing
the current segment up and received any WAL data that should be
written into the next segment. If walreceiver exited just before
any WAL data in the next segment arrived at standby, it did not
create the archive notification file of the current segment
even though that's known completed. This behavior could cause
WAL archiving of the segment to be delayed until subsequent
restartpoints or checkpoints created its notification file.
To fix the issue, this commit changes walreceiver so that it creates
an archive notification file of a current WAL segment immediately
if that's known completed before receiving next WAL data.
Back-patch to all supported branches.

WAL records may span multiple segments, but XLogWrite() does not
wait for the entire record to be written out to disk before
creating archive status files. Instead, as soon as the last WAL page of
the segment is written, the archive status file is created, and the
archiver may process it. If PostgreSQL crashes before it is able to
write and flush the rest of the record (in the next WAL segment), the
wrong version of the first segment file lingers in the archive, which
causes operations such as point-in-time restores to fail.
To fix this, keep track of records that span across segments and ensure
that segments are only marked ready-for-archival once such records have
been completely written to disk.
This has always been wrong, so backpatch all the way back.

To add support for streaming transactions at prepare time into the
built-in logical replication, we need to do the following things:
* Modify the output plugin (pgoutput) to implement the new two-phase API
callbacks, by leveraging the extended replication protocol.
* Modify the replication apply worker, to properly handle two-phase
transactions by replaying them on prepare.
* Add a new SUBSCRIPTION option "two_phase" to allow users to enable
two-phase transactions. We enable the two_phase once the initial data sync
is over.
We however must explicitly disable replication of two-phase transactions
during replication slot creation, even if the plugin supports it. We
don't need to replicate the changes accumulated during this phase,
and moreover, we don't have a replication connection open so we don't know
where to send the data anyway.
The streaming option is not allowed with this new two_phase option. This
can be done as a separate patch.
We don't allow to toggle two_phase option of a subscription because it can
lead to an inconsistent replica. For the same reason, we don't allow to
refresh the publication once the two_phase is enabled for a subscription
unless copy_data option is false.

Before a restartpoint finishes PreallocXlogFiles(), a startup process
KeepFileRestoredFromArchive() call can unlink the preallocated segment.
If a CHECKPOINT sql command had elicited the restartpoint experiencing
the race condition, that sql command failed. Moreover, the restartpoint
omitted its log_checkpoints message and some inessential resource
reclamation. Prevent the ERROR by skipping open() of the segment.
Since these consequences are so minor, no back-patch.

I started out with the goal of reporting ERRCODE_CONNECTION_FAILURE
when walrcv_connect() fails, but as I looked around I realized that
whoever wrote this code was of the opinion that errcodes are purely
optional. That's not my understanding of our project policy. Hence,
make sure that an errcode is provided in each ereport that (a) is
ERROR or higher level and (b) isn't arguably an internal logic error.

While this is not per policy, it's also largely cosmetic, since few
of these cases could get reported to applications. So I don't
feel a need to back-patch.

The only change worthy of note is that pgindent messed up the formatting
of launcher.c's struct LogicalRepWorkerId, which led me to notice that
that struct wasn't used at all anymore, so I just took it out.
