Add reformatting and rename commits to .git-blame-ignore-revs.

The file has only ever held the commits that were added as they happened, so reformatting done before it existed, and reformatting nobody thought to add at the time, still stands between a line and whoever wrote it. Thirty-five commits are added, the earliest from 2018.

The candidates were found by running git log --numstat over the whole history twice, once plain and once with -w, and comparing the two. A commit whose churn falls away when whitespace is ignored changed nothing but whitespace. The double space commits already in the file came back out of that scan, which is how the check was calibrated.

Mechanical changes that are not whitespace are added as well. A rename carried through every file that used the name, a parameter added to or dropped from a macro everywhere it is called, a define moved to a header: each rewrites every line it touches without changing what any of them do, which leaves blame in the state a reformat leaves it.

Commits that only touched files no longer in the tree were left out since ignoring them buys nothing. Most are from the Perl era or are expect logs, and the largest changed the spacing of 49537 lines that have since been deleted.
