When lower_case_table_names > 0 the db part of table names are
downcased in CREATE TABLE or SHOW CREATE TABLE (in
TABLE_LIST::TABLE_LIST). This patch makes ALTER do the same, which
fixes assertion failure caused the inconsistency.
