Jacob Champion 2c70ed7562 ProxyFCGISetEnvIf: reject invalid invocations, streamline unsets 
Neither of the following makes sense:
- ProxyFCGISetEnvIf cond !VARIABLE value
- ProxyFCGISetEnvIf cond !

Error out in these cases. Also, don't execute the (unused) replacement
expression when unsetting.
