as test certificates just allow people who don't know what they're doing
to dig a bigger hole for themselves and don't really help anybody who
DOES know what they're doing much. So all of the documentation references
to "make certificate" are now removed.

Change absolute self-references to relative jumps.
TODO:
* IMHO the topic ToC3 should be deleted completely.
* All references to "patch Apache" or "EAPI patch" should be removed
* A native english speaker might want to proof-read the doc and polish it

I also got rid of the old cover page and the overview, since they seemed
obsolete.
