Update css to have permalink more in line with httpd.a.o:
- default to white (blue on dark-blue is not really visible)
- visible only when the mouse is over the corresponding heading

This page pulls in directives that declare an <override> element and
groups them by class. The documentation for each class comes from
overrides.xml. Any undocumented classes are flagged with a boilerplate
fallback text (this situation is only likely to occur with a misspelling
or otherwise invalid <override> somewhere else in the documentation).

disturbing to the flow of text. Here I replace it with a dashed-unline.
Also, allow the omission of the ref attribute where the glossary term is
exactly equal to the link contents.

- reintroduced the sidebar for the module index page (obvious ;-)
* didn't want to leave the section links so alone, thus added some
<seealso>s.
- changed the ordering of the core/MPMs as follows:
* core
* mpm_common
* alphabetical list of the mpms (ordered by their "natural" names).
- same appeared on the sitemap
- changed <h1> headings of the modulesynopsis files, so that they match the
titles in the sitemap. This should better reflect the nature of the
modules. (affects only core & MPMs)

More tweaking welcome.
1. Lower the main text font size by 1 pixel and the <example> font
size by 2 pixels.
2. Remove the font-weight specified in <code> because it was
preventing bold code from working.
3. Try out a funny shade of brown for <module>. What'd'ya think?
