902fbd0c27 Docs update concerning recent htpasswd and htdbm 
additions.

Add some cross refernces between dbmmanage and htdbm.

Update man page for htdbm and htpasswd.

Add bcrypt info to misc/password_encryptions.

5ed1619e1d Change the default algorithm for htpasswd to MD5 on all platforms. Crypt 
with its 8 character limit is not useful anymore.
