e62f32eeee Remove out-of-date details about which modules are enabled by default 
from the documentation, in favor of suggesting getting the up-to-date
information by running ./configure -h.  

The special cases previously documented here (such as, this module
depends on this other module, or you probably don't want this module
unless you're a developer), have had that information added to the
output of configure -h in r1029814.

 - Win32 Build -- I didn't want to muck with .dsp files.
 - Netware Build - Same Story as Win32.

e0eb208c1d Addition of htdbm documentation. nroff was meant to be generated, 
but wasn't.  I suspect deps.xml in build/ needs to be modified, but
could someone else with more of a clue assist here, thanks.

b13c96a785 --with-module can now take more than one module to be statically 
linked: --with-module=<modtype>:<modfile>,<modtype>:<modfile>,...
If the <modtype>-subdirectory doesn't exist it will be created and
populated with a standard Makefile.in.

f4887a358b Intergrate Kess' configure docs in the docs tree and, 
in particular, integrate them into install.xml by removing
large chunks of that document and making it a summary
with pointers into configure.xml.

(I also snuck in the odd fix to both install.xml and
configure.xml.  Sorry for the mix.)
