Cleanups, 1 bug fix:

- HTMLparser.c: fixed htmlStartCloseIndexinitialized init
- entities.h: exported xmlInitializePredefinedEntities
- parser.[ch] : added xmlInitParser()
- parserInternals.h : had to export htmlInitAutoClose()
