cleanup cleanup too added xmlNoNetExternalEntityLoader() from xsltproc

* Makefile.am: cleanup
* threads.c: cleanup too
* xmlIO.c include/libxml/xmlIO.h: added xmlNoNetExternalEntityLoader()
  from xsltproc
* include/libxml/tree.h include/libxml/parser.h: trying to break a
  dependancy loop.
