Viktor Szakats e4f8d1fbdc src: rename and tidy up digest length macros ...

To allow definining them globally, and avoid reusing macros names
originally defined by OpenSSL.

- rename `<name>_DIGEST_LENGTH` to `SSH2_<name>_DIG_LEN`.
  To move to the libssh2 namespace.
- rename `SSH2_SHA_DIG_LEN` to `SSH2_SHA1_DIG_LEN`,
  and drop no longer necessary local trick from kex.c.
- deduplicate definitions into `crypto.h`.
- add `SSH2_RIPEMD160_DIG_LEN` and use it.
- mac: replace magic numbers with these macros, where missing.
