Deprecated since v1.2.0 (2009-08-10).

Also:
- disable examples ssh2_echo and x11 for no-deprecate builds. They
  will need to be rewritten or deleted due to reliance on this API.
- ssh2_echo: replace a macro with `sizeof()`.
- update man page.
