README: Mention CONTRIBUTING not SubmittingPatches

Browse Source 
The “SubmittingPatches” file is no more since commit
a76badf77af9ff92164fd97327d63cc731d753ef, but the READMEs were still
referencing it.
They now correctly point to “CONTRIBUTING.md”.
