Bartosz Golaszewski 64fde2647d README: move SPDX boiler-plate into REUSE.toml

There's no reason to keep the SPDX license and copyright headers in the
.md documents (README, CHANGELOG, etc.) as it only decreases their
readability if one doesn't use any rendering tool. Move all SPDX headers
for .md documents into REUSE.toml.
