* doc/Makefile (undocumented-modules.texi): New rule.
(%.info, %.html, %.dvi, %.pdf): Depend on undocumented-modules.texi.
(mostlyclean): Remove also *.m and *.tmp.
(force): New rule.
* doc/*.texi: Add module index entries.
* doc/*/*.texi: Likewise.
