- removed the u-law codes...

- by default sound converted to stereo, and there is a --mono switch
- if there is no sound for a frame in fmf, fmfconv add silence
- there is a --sound-only switch, when fmfconv look sound output file.
e.g. you can use fmfconv --sound-only -w fmf.fmf fmf.wav instead of fmfconv
-w fmf.fmf /dev/null fmf.wav
in fmfconv_ff.c:
- change the deprecated av_get_bits_per_sample_format() to
av_get_bits_per_sample_fmt() if available
- fix resample context reinit
