Prevent creating namespaces/orgs on pushes (CREATE_NAMESPACE_ON_PUSH)
if user is restricted.

Also updates RESTRICTED_USERS_WHITELIST to defaults to all if not set,
given that FEATURE_RESTRICTED_USERS is set.
