8f941bc7a1 Add asserts to detect if a transaction commits without first incrementing 
the transaction counter.  These asserts are intended to prevent future
problems similar to ticket #3584. (CVS 6179)

47c3b3e099 When compiled with SQLITE_DEBUG (in other words, when assert() is enabled) 
the ALWAYS and NEVER macros will throw an assertion if their arguments are
not true or false, respectively. (CVS 6159)

dd5f5a6231 Multi-index OR-clause optimization now works for simple tests. There are 
no test scripts for it yet, though.  And it is disabled by default, pending
further testing and optimization.  We need a lot of both. (CVS 6058)

ea6788322e Never use strlen(). Use our own internal sqlite3Strlen30() which is 
guaranteed to never overflow an integer.  Additional explicit casts to
avoid nuisance warning messages. (CVS 6007)

2bf90f1e64 A subquery in the result set of a SELECT should be able to look into outer 
queries to find variable references.  But a subquery in the FROM clause
should not be able to do this.  Ticket #3530. (CVS 5998)

d1370b6d92 Remove the vestigial mem4 and mem6 memory allocators. Add the 
SQLITE_ZERO_MALLOC compile-time option and the mem0.c module to

e82f5d04c3 Raise the hard upper bound on SQLITE_MAX_FUNCTION_ARG to 1000 from 100. 
The default upper bound is still 100, but it can now be raised as high
as 1000 at compile-time. (CVS 5780)

18472fa7b8 Remove the SQLITE_MUTEX_APPDEF compile-time option. The SQLITE_THREADSAFE=0 
option always removes all mutex code.  For application-defined mutexes only,
use SQLITE_THREADSAFE=1 with SQLITE_MUTEX_NOOP=1.  Ticket #3421. (CVS 5779)
