Add the xIntegrity method to the sqlite3_module object. Implement this

Enable DEFENSIVE mode by default for tests. This requires lots of case of

turning DEFENSIVE off in order to dodgy things to the database for testing
purposes. No all of those cases are yet handled, so "make test" does not run
to completion.

Have fts5 store rowids in ascending order. Query speed is virtually the same regardless of rowid order, and ascending order makes some insert optimizations easier.
