Bruce Momjian e72821ca08 doc: restructure ALTER DEFAULT PRIVILEGES ...

Clarify that default privileges are not inherited and reorder
paragraphs.  This is a follow up to a recent ALTER DEFAULT PRIVILEGES
doc patch.
