DocBook XML is superficially compatible with DocBook SGML but has a slightly stricter DTD that we have been violating in a few cases. Although XSLT doesn't care whether the document is valid, the style sheets don't necessarily process invalid documents correctly, so we need to work toward fixing this. This first commit moves the indexterms in refentry elements to an allowed position. It has no impact on the output.

<title>Description</title>
<para>
<command>ALTER OPERATOR</command> changes the definition of
an operator. The only currently available functionality is to change the
owner of the operator.
</para>
<para>
You must own the operator to use <command>ALTER OPERATOR</>.
To alter the owner, you must also be a direct or indirect member of the new
owning role, and that role must have <literal>CREATE</literal> privilege on
the operator's schema. (These restrictions enforce that altering the owner
doesn't do anything you couldn't do by dropping and recreating the operator.
However, a superuser can alter ownership of any operator anyway.)
