Noah Misch cb2c696b1d Fix nonstandard SQL command doc id= values introduced in the v19 cycle. 
While some <refentry> instances of supported PostgreSQL versions use the
sql-alter-table style, sql-altertable is the dominant style.  Change
these before they're baked into the doc URLs of a supported version.
