1336bd9862 pg_buffercache: Allow huge allocations.
Otherwise, users who have configured shared_buffers >= 256GB won't
be able to use this module.  There probably aren't many of those, but
it doesn't hurt anything to fix it so that it works.

Backpatch to 9.4, where MemoryContextAllocHuge was introduced.  The
same problem exists in older branches, but there's no easy way to
fix it there.

e7128e8dbb Create function prototype as part of PG_FUNCTION_INFO_V1 macro
Because of gcc -Wmissing-prototypes, all functions in dynamically
loadable modules must have a separate prototype declaration.  This is
meant to detect global functions that are not declared in header files,
but in cases where the function is called via dfmgr, this is redundant.
Besides filling up space with boilerplate, this is a frequent source of
compiler warnings in extension modules.

We can fix that by creating the function prototype as part of the
PG_FUNCTION_INFO_V1 macro, which such modules have to use anyway.  That
makes the code of modules cleaner, because there is one less place where
the entry points have to be listed, and creates an additional check that
functions have the right prototype.

Remove now redundant prototypes from contrib and other modules.

ea9df812d8 Relax the requirement that all lwlocks be stored in a single array.
This makes it possible to store lwlocks as part of some other data
structure in the main shared memory segment, or in a dynamic shared
memory segment.  There is still a main LWLock array and this patch does
not move anything out of it, but it provides necessary infrastructure
for doing that in the future.

This change is likely to increase the size of LWLockPadded on some
platforms, especially 32-bit platforms where it was previously only
16 bytes.

c219d9b0a5 Split tuple struct defs from htup.h to htup_details.h
This reduces unnecessary exposure of other headers through htup.h, which
is very widely included by many files.

I have chosen to move the function prototypes to the new file as well,
because that means htup.h no longer needs to include tupdesc.h.  In
itself this doesn't have much effect in indirect inclusion of tupdesc.h
throughout the tree, because it's also required by execnodes.h; but it's
something to explore in the future, and it seemed best to do the htup.h
change now while I'm busy with it.

2a275e6d3c Fix pg_buffercache to release buffer partition locks in reverse order,
and add a note about why.  This is not tremendously important right now,
probably, but it will get more urgent if NUM_BUFFER_PARTITIONS is increased
as much as proposed.

10b9ca3d05 Split the buffer mapping table into multiple separately lockable
partitions, as per discussion.  Passes functionality checks, but

07eeb9d109 Do all accesses to shared buffer headers through volatile-qualified
pointers, to ensure that compilers won't rearrange accesses to occur
while we're not holding the buffer header spinlock.  It's probably
not necessary to mark volatile in every single place in bufmgr.c,
but better safe than sorry.  Per trouble report from Kevin Grittner.
