d3ae2a24f2 Add allow_alter_system GUC.
This is marked PGC_SIGHUP, so it can only be set in a configuration
file, not anywhere else; and it is also marked GUC_DISALLOW_IN_AUTO_FILE,
so it can't be set using ALTER SYSTEM. When set to false, the
ALTER SYSTEM command is disallowed.

There was considerable concern that this would be misinterpreted as
a security feature, which it is not, because a determined superuser
has various ways of bypassing it. Hence, a lot of work has gone into
wordsmithing the documentation, in the hopes of avoiding any such
confusion.

a0ffa885e4 Allow granting SET and ALTER SYSTEM privileges on GUC parameters.
This patch allows "PGC_SUSET" parameters to be set by non-superusers
if they have been explicitly granted the privilege to do so.
The privilege to perform ALTER SYSTEM SET/RESET on a specific parameter
can also be granted.
Such privileges are cluster-wide, not per database.  They are tracked
in a new shared catalog, pg_parameter_acl.

Granting and revoking these new privileges works as one would expect.
One caveat is that PGC_USERSET GUCs are unaffected by the SET privilege
--- one could wish that those were handled by a revocable grant to
PUBLIC, but they are not, because we couldn't make it robust enough
for GUCs defined by extensions.

3c49c6facb Convert documentation to DocBook XML
Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.

The source files are still named *.sgml, but they are actually XML files
now.  Renaming could be considered later.

In the build system, the intermediate step to convert from SGML to XML
is removed.  Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.

The documentation toolchain instructions are updated and are much
simpler now.

1ff01b3902 Convert SGML IDs to lower case
IDs in SGML are case insensitive, and we have accumulated a mix of upper
and lower case IDs, including different variants of the same ID.  In
XML, these will be case sensitive, so we need to fix up those
differences.  Going to all lower case seems most straightforward, and
the current build process already makes all anchors and lower case
anyway during the SGML->XML conversion, so this doesn't create any
difference in the output right now.  A future XML-only build process
would, however, maintain any mixed case ID spellings in the output, so
that is another reason to clean this up beforehand.

c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

b555ed8102 Merge wal_level "archive" and "hot_standby" into new name "replica"
The distinction between "archive" and "hot_standby" existed only because
at the time "hot_standby" was added, there was some uncertainty about
stability.  This is now a long time ago.  We would like to move forward
with simplifying the replication configuration, but this distinction is
in the way, because a primary server cannot tell (without asking a
standby or predicting the future) which one of these would be the
appropriate level.

Pick a new name for the combined setting to make it clearer that it
covers all (non-logical) backup and replication uses.  The old values
are still accepted but are converted internally.

af06aa822d Improve documentation around parameter-setting and ALTER SYSTEM.
The ALTER SYSTEM ref page hadn't been held to a very high standard, nor
was the feature well integrated into section 18.1 (parameter setting).
Also, though commit 4c4654afe had improved the structure of 18.1, it also
introduced a lot of poor wording, imprecision, and outright falsehoods.
Try to clean that up.

bd3b7a9eef Support ALTER SYSTEM RESET command.
This patch allows us to execute ALTER SYSTEM RESET command to
remove the configuration entry from postgresql.auto.conf.

9ba78fb0b9 Don't allow data_directory to be set in postgresql.auto.conf by ALTER SYSTEM.
data_directory could be set both in postgresql.conf and postgresql.auto.conf so far.
This could cause some problematic situations like circular definition. To avoid such
situations, this commit forbids a user to set data_directory in postgresql.auto.conf.

Backpatch this to 9.4 where ALTER SYSTEM command was introduced.

bb4eefe7bf doc: Improve DocBook XML validity
DocBook XML is superficially compatible with DocBook SGML but has a
slightly stricter DTD that we have been violating in a few cases.
Although XSLT doesn't care whether the document is valid, the style
sheets don't necessarily process invalid documents correctly, so we need
to work toward fixing this.

This first commit moves the indexterms in refentry elements to an
allowed position.  It has no impact on the output.
