Support assignment to subfields of composite columns in UPDATE and INSERT.
...
As a side effect, cause subscripts in INSERT targetlists to do something
more or less sensible; previously we evaluated such subscripts and then
effectively ignored them. Another side effect is that UPDATE-ing an
element or slice of an array value that is NULL now produces a non-null
result, namely an array containing just the assigned-to positions.

This patch includes a lot of minor cleanups to the SGML documentation,
...
including:
- replacing all the appropriate usages of <citetitle>PostgreSQL
...</citetitle> with &cite-user;, &cite-admin;, and so on
- fix an omission in the EXECUTE documentation
- add some more text to the EXPLAIN documentation
- improve the PL/PgSQL RETURN NEXT documentation (more work to do here)
- minor markup fixes

Make UPDATE and DELETE privileges distinct. Add REFERENCES and TRIGGER

privileges. INSERT and COPY FROM now require INSERT (only). Add

Fix markup for docbook2man man page generation.

No big deal; fixed lots of other markup at the same time.
Bigest change: make sure there is no whitespace
in front of <term> contents.
This will probably help the other output types too.
