b3817f5f77 Improve hash_create()'s API for some added robustness.
Invent a new flag bit HASH_STRINGS to specify C-string hashing, which
was formerly the default; and add assertions insisting that exactly
one of the bits HASH_STRINGS, HASH_BLOBS, and HASH_FUNCTION be set.
This is in hopes of preventing recurrences of the type of oversight
fixed in commit a1b8aa1e4 (i.e., mistakenly omitting HASH_BLOBS).

Also, when HASH_STRINGS is specified, insist that the keysize be
more than 8 bytes.  This is a heuristic, but it should catch
accidental use of HASH_STRINGS for integer or pointer keys.
(Nearly all existing use-cases set the keysize to NAMEDATALEN or
more, so there's little reason to think this restriction should
be problematic.)

Tweak hash_create() to insist that the HASH_ELEM flag be set, and
remove the defaults it had for keysize and entrysize.  Since those
defaults were undocumented and basically useless, no callers
omitted HASH_ELEM anyway.

Also, remove memset's zeroing the HASHCTL parameter struct from
those callers that had one.  This has never been really necessary,
and while it wasn't a bad coding convention it was confusing that
some callers did it and some did not.  We might as well save a few
cycles by standardizing on "not".

Also improve the documentation for hash_create().

In passing, improve reinit.c's usage of a hash table by storing
the key as a binary Oid rather than a string; and, since that's
a temporary hash table, allocate it in CurrentMemoryContext for
neatness.

be0a666665 Remove large fill factor support from dynahash.c.
Since ancient times we have had support for a fill factor (maximum load
factor) to be set for a dynahash hash table, but:

1.  It was an integer, whereas for in-memory hash tables interesting
load factor targets are probably somewhere near the 0.75-1.0 range.

2.  It was implemented in a way that performed an expensive division
operation that regularly showed up in profiles.

3.  We are not aware of anyone ever having used a non-default value.

Therefore, remove support, effectively fixing it at 1.

9341c783cc Put all the prototypes for hashfn.c into the same header file.
Previously, some of the prototypes for functions in hashfn.c were
in utils/hashutils.h and others were in utils/hsearch.h, but that
is confusing and has no particular benefit.

Patch by me, reviewed by Suraj Kharage and Mark Dilger.

07b95c3d83 Move bitmap_hash and bitmap_match to bitmapset.c.
The closely-related function bms_hash_value is already defined in that
file, and this change means that hashfn.c no longer needs to depend on
nodes/bitmapset.h. That gets us closer to allowing use of the hash
functions in hashfn.c in frontend code.

8255c7a5ee Phase 2 pgindent run for v12.
Switch to 2.1 version of pg_bsd_indent.  This formats
multiline function declarations "correctly", that is with
additional lines of parameter declarations indented to match
where the first line's left parenthesis is.

c7b8998ebb Phase 2 of pgindent updates.
Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.

Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code.  The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there.  BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs.  So the
net result is that in about half the cases, such comments are placed
one tab stop left of before.  This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.

Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.

This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

e3860ffa4d Initial pgindent run with pg_bsd_indent version 2.0.
The new indent version includes numerous fixes thanks to Piotr Stefaniak.
The main changes visible in this commit are:

* Nicer formatting of function-pointer declarations.
* No longer unexpectedly removes spaces in expressions using casts,
  sizeof, or offsetof.
* No longer wants to add a space in "struct structname *varname", as
  well as some similar cases for const- or volatile-qualified pointers.
* Declarations using PG_USED_FOR_ASSERTS_ONLY are formatted more nicely.
* Fixes bug where comments following declarations were sometimes placed
  with no space separating them from the code.
* Fixes some odd decisions for comments following case labels.
* Fixes some cases where comments following code were indented to less
  than the expected column 33.

On the less good side, it now tends to put more whitespace around typedef
names that are not listed in typedefs.list.  This might encourage us to
put more effort into typedef name collection; it's not really a bug in
indent itself.

There are more changes coming after this round, having to do with comment
indentation and alignment of lines appearing within parentheses.  I wanted
to limit the size of the diffs to something that could be reviewed without
one's eyes completely glazing over, so it seemed better to split up the
changes as much as practical.

4a14f13a0a Improve hash_create's API for selecting simple-binary-key hash functions.
Previously, if you wanted anything besides C-string hash keys, you had to
specify a custom hashing function to hash_create().  Nearly all such
callers were specifying tag_hash or oid_hash; which is tedious, and rather
error-prone, since a caller could easily miss the opportunity to optimize
by using hash_uint32 when appropriate.  Replace this with a design whereby
callers using simple binary-data keys just specify HASH_BLOBS and don't
need to mess with specific support functions.  hash_create() itself will
take care of optimizing when the key size is four bytes.

This nets out saving a few hundred bytes of code space, and offers
a measurable performance improvement in tidbitmap.c (which was not
exploiting the opportunity to use hash_uint32 for its 4-byte keys).
There might be some wins elsewhere too, I didn't analyze closely.

In future we could look into offering a similar optimized hashing function
for 8-byte keys.  Under this design that could be done in a centralized
and machine-independent fashion, whereas getting it right for keys of
platform-dependent sizes would've been notationally painful before.

For the moment, the old way still works fine, so as not to break source
code compatibility for loadable modules.  Eventually we might want to
remove tag_hash and friends from the exported API altogether, since there's
no real need for them to be explicitly referenced from outside dynahash.c.

2065dd2834 Prevent very-low-probability PANIC during PREPARE TRANSACTION.
The code in PostPrepare_Locks supposed that it could reassign locks to
the prepared transaction's dummy PGPROC by deleting the PROCLOCK table
entries and immediately creating new ones.  This was safe when that code
was written, but since we invented partitioning of the shared lock table,
it's not safe --- another process could steal away the PROCLOCK entry in
the short interval when it's on the freelist.  Then, if we were otherwise
out of shared memory, PostPrepare_Locks would have to PANIC, since it's
too late to back out of the PREPARE at that point.

Fix by inventing a dynahash.c function to atomically update a hashtable
entry's key.  (This might possibly have other uses in future.)

This is an ancient bug that in principle we ought to back-patch, but the
odds of someone hitting it in the field seem really tiny, because (a) the
risk window is small, and (b) nobody runs servers with maxed-out lock
tables for long, because they'll be getting non-PANIC out-of-memory errors
anyway.  So fixing it in HEAD seems sufficient, at least until the new
code has gotten some testing.

a2e923a652 Fix dynahash.c to suppress hash bucket splits while a hash_seq_search() scan
is in progress on the same hashtable.  This seems the least invasive way to
fix the recently-recognized problem that a split could cause the scan to
visit entries twice or (with much lower probability) miss them entirely.
The only field-reported problem caused by this is the "failed to re-find
shared lock object" PANIC in COMMIT PREPARED reported by Michel Dorochevsky,
which was caused by multiply visited entries.  However, it seems certain
that mdsync() is vulnerable to missing required fsync's due to missed
entries, and I am fearful that RelationCacheInitializePhase2() might be at
risk as well.  Because of that and the generalized hazard presented by this
bug, back-patch all the supported branches.

Along the way, fix pg_prepared_statement() and pg_cursor() to not assume
that the hashtables they are examining will stay static between calls.
This is risky regardless of the newly noted dynahash problem, because
hash_seq_search() has never promised to cope with deletion of table entries
other than the just-returned one.  There may be no bug here because the only
supported way to call these functions is via ExecMakeTableFunctionResult()
which will cycle them to completion before doing anything very interesting,
but it seems best to get rid of the assumption.  This affects 8.2 and HEAD
only, since those functions weren't there earlier.

to the low-order bits of the entry hash value.  Also make some incidental

2924ca6747 Tweak dynahash.c to avoid wasting memory space in non-shared hash tables.
palloc() will normally round allocation requests up to the next power of 2,
so make dynahash choose allocation sizes that are as close to a power of 2
as possible.
Back-patch to 8.1 --- the problem exists further back, but a much larger
patch would be needed and it doesn't seem worth taking any risks.

0007490e09 Convert the arithmetic for shared memory size calculation from 'int'
to 'Size' (that is, size_t), and install overflow detection checks in it.
This allows us to remove the former arbitrary restrictions on NBuffers
etc.  It won't make any difference in a 32-bit machine, but in a 64-bit
machine you could theoretically have terabytes of shared buffers.
(How efficiently we could manage 'em remains to be seen.)  Similarly,
num_temp_buffers, work_mem, and maintenance_work_mem can be set above
2Gb on a 64-bit machine.  Original patch from Koichi Suzuki, additional
work by moi.

06ae88a82e Tweak dynahash.c to not allocate so many entries at once when dealing
with a table that has a small predicted size.  Avoids wasting several
hundred K on the timezone hash table, which is likely to have only one
or a few entries, but the entries use up 10Kb apiece ...

6a6f2d91d4 When using C-string lookup keys in a dynahash.c hash table, use strncpy()
not memcpy() to copy the offered key into the hash table during HASH_ENTER.
This avoids possible core dump if the passed key is located very near the
end of memory.  Per report from Stefan Kaltenbrunner.

e92a88272e Modify hash_search() API to prevent future occurrences of the error
spotted by Qingqing Zhou.  The HASH_ENTER action now automatically
fails with elog(ERROR) on out-of-memory --- which incidentally lets
us eliminate duplicate error checks in quite a bunch of places.  If
you really need the old return-NULL-on-out-of-memory behavior, you
can ask for HASH_ENTER_NULL.  But there is now an Assert in that path
checking that you aren't hoping to get that behavior in a palloc-based
hash table.
Along the way, remove the old HASH_FIND_SAVE/HASH_REMOVE_SAVED actions,
which were not being used anywhere anymore, and were surely too ugly
and unsafe to want to see revived again.

055467d504 Marginal hack to use a specialized hash function for dynahash hashtables
whose keys are OIDs.  The only one that looks particularly performance
critical is the relcache hashtable, but as long as we've got the function
we may as well use it wherever it's applicable.

80860c32d9 Improve dynahash.c's API so that caller can specify the comparison function
as well as the hash function (formerly the comparison function was hardwired
as memcmp()).  This makes it possible to eliminate the special-purpose
hashtable management code in execGrouping.c in favor of using dynahash to
manage tuple hashtables; which is a win because dynahash knows how to expand
a hashtable when the original size estimate was too small, whereas the
special-purpose code was too stupid to do that.  (See recent gripe from
Stephan Szabo about poor performance when hash table size estimate is way
off.)  Free side benefit: when using string_hash, the default comparison
function is now strncmp() instead of memcmp().  This should eliminate some
part of the overhead associated with larger NAMEDATALEN values.
