3bf31dd243 Do a tiny bit of header file maintenance
Stop including utils/relcache.h in access/genam.h, and stop including
htup_details.h in nodes/tidbitmap.h. Both these files (genam.h and
tidbitmap.h) are widely used in other header files, so it's in our best
interest that they remain as lean as reasonable.

dbbca2cf29 Remove unused #include's from backend .c files
as determined by include-what-you-use (IWYU)

While IWYU also suggests to *add* a bunch of #include's (which is its
main purpose), this patch does not do that. In some cases, a more
specific #include replaces another less specific one.

Some manual adjustments of the automatic result:

- IWYU currently doesn't know about includes that provide global
  variable declarations (like -Wmissing-variable-declarations), so
those includes are being kept manually.

- All includes for port(ability) headers are being kept for now, to
  play it safe.

- No changes of catalog/pg_foo.h to catalog/pg_foo_d.h, to keep the
  patch from exploding in size.

Note that this patch touches just *.c files, so nothing declared in
header files changes in hidden ways.

As a small example, in src/backend/access/transam/rmgr.c, some IWYU
pragma annotations are added to handle a special case there.

d952373a98 New header varatt.h split off from postgres.h
This new header contains all the variable-length data types support
(TOAST support) from postgres.h, which isn't needed by large parts of
the backend code.

8255c7a5ee Phase 2 pgindent run for v12.
Switch to 2.1 version of pg_bsd_indent. This formats
multiline function declarations "correctly", that is with
additional lines of parameter declarations indented to match
where the first line's left parenthesis is.

352a24a1f9 Generate fmgr prototypes automatically
Gen_fmgrtab.pl creates a new file fmgrprotos.h, which contains
prototypes for all functions registered in pg_proc.h. This avoids
having to manually maintain these prototypes across a random variety of
header files. It also automatically enforces a correct function
signature, and since there are warnings about missing prototypes, it
will detect functions that are defined but not registered in
pg_proc.h (or otherwise used).

32909a57f9 Fix network_spgist.c build failures from missing AF_INET definition.
AF_INET is apparently defined in something that's pulled in automatically
on Linux, but the buildfarm says that's not true everywhere. Comparing
to network_gist.c suggests that including <sys/socket.h> ought to fix it,
and the POSIX standard concurs.

77e2906821 Create an SP-GiST opclass for inet/cidr.
This seems to offer significantly better search performance than the
existing GiST opclass for inet/cidr, at least on data with a wide mix
of network mask lengths. (That may suggest that the data splitting
heuristics in the GiST opclass could be improved.)
