cdbc0ca48c Fix background workers for EXEC_BACKEND
Commit da07a1e8 was broken for EXEC_BACKEND because I failed to realize
that the MaxBackends recomputation needed to be duplicated by
subprocesses in SubPostmasterMain.  However, instead of having the value
be recomputed at all, it's better to assign the correct value at
postmaster initialization time, and have it be propagated to exec'ed
backends via BackendParameters.

MaxBackends stays as zero until after modules in
shared_preload_libraries have had a chance to register bgworkers, since
the value is going to be untrustworthy till that's finished.

da07a1e856 Background worker processes
Background workers are postmaster subprocesses that run arbitrary
user-specified code.  They can request shared memory access as well as
backend database connections; or they can just use plain libpq frontend
database connections.

Modules listed in shared_preload_libraries can register background
workers in their _PG_init() function; this is early enough that it's not
necessary to provide an extra GUC option, because the necessary extra
resources can be allocated early on.  Modules can install more than one
bgworker, if necessary.

Care is taken that these extra processes do not interfere with other
postmaster tasks: only one such process is started on each ServerLoop
iteration.  This means a large number of them could be waiting to be
started up and postmaster is still able to quickly service external
connection requests.  Also, shutdown sequence should not be impacted by
a worker process that's reasonably well behaved (i.e. promptly responds
to termination signals.)

The current implementation lets worker processes specify their start
time, i.e. at what point in the server startup process they are to be
started: right after postmaster start (in which case they mustn't ask
for shared memory access), when consistent state has been reached
(useful during recovery in a HOT standby server), or when recovery has
terminated (i.e. when normal backends are allowed).

In case of a bgworker crash, actions to take depend on registration
data: if shared memory was requested, then all other connections are
taken down (as well as other bgworkers), just like it were a regular
backend crashing.  The bgworker itself is restarted, too, within a
configurable timeframe (which can be configured to be never).

More features to add to this framework can be imagined without much
effort, and have been discussed, but this seems good enough as a useful
unit already.

An elementary sample module is supplied.

5d8a894e30 Cancel running query if it is detected that the connection to the client is
lost. The only way we detect that at the moment is when write() fails when
we try to write to the socket.

9d3b502443 Improve logging of autovacuum I/O activity
This adds some I/O stats to the logging of autovacuum (when the
operation takes long enough that log_autovacuum_min_duration causes it
to be logged), so that it is easier to tune.  Notably, it adds buffer
I/O counts (hits, misses, dirtied) and read and write rate.

76dd09bbec Add postmaster/postgres undocumented -b option for binary upgrades.
This option turns off autovacuum, prevents non-super-user connections,
and enables oid setting hooks in the backend.  The code continues to use
the old autoavacuum disable settings for servers with earlier catalog
versions.

This includes a catalog version bump to identify servers that support
the -b option.

cc1ed40d57 Object access hook framework, with post-creation hook.
After a SQL object is created, we provide an opportunity for security
or logging plugins to get control; for example, a security label provider
could use this to assign an initial security label to newly created
objects.  The basic infrastructure is (hopefully) reusable for other types
of events that might require similar treatment.

00e6a16d01 Change the autovacuum launcher to read pg_database directly, rather than
via the "flat files" facility.  This requires making it enough like a backend
to be able to run transactions; it's no longer an "auxiliary process" but
more like the autovacuum worker processes.  Also, its signal handling has
to be brought into line with backends/workers.  In particular, since it
now has to handle procsignal.c processing, the special autovac-launcher-only
signal conditions are moved to SIGUSR2.

0a00c9a8ef Remove useless code that propagated FrontendProtocol to a backend via a
PostgresMain switch.  In point of fact, FrontendProtocol is already set
in a backend process, since ProcessStartupPacket() is executed inside
the backend --- it hasn't been run by the postmaster for many years.
And if it were, we'd still certainly want FrontendProtocol to be set before
we get as far as PostgresMain, so that startup errors get reported in the
right protocol.

-v might have some future use in standalone backends, so I didn't go so
far as to remove the switch outright.

Also, initialize FrontendProtocol to 0 not PG_PROTOCOL_LATEST.  The only
likely result of presetting it like that is to mask failure-to-set-it
mistakes.

969d7cd431 Install a "dead man switch" to allow the postmaster to detect cases where
a backend has done exit(0) or exit(1) without having disengaged itself
from shared memory.  We are at risk for this whenever third-party code is
loaded into a backend, since such code might not know it's supposed to go
through proc_exit() instead.  Also, it is reported that under Windows
there are ways to externally kill a process that cause the status code
returned to the postmaster to be indistinguishable from a voluntary exit
(thank you, Microsoft).  If this does happen then the system is probably
hosed --- for instance, the dead session might still be holding locks.
So the best recovery method is to treat this like a backend crash.

The dead man switch is armed for a particular child process when it
acquires a regular PGPROC, and disarmed when the PGPROC is released;
these should be the first and last touches of shared memory resources
in a backend, or close enough anyway.  This choice means there is no
coverage for auxiliary processes, but I doubt we need that, since they
shouldn't be executing any user-provided code anyway.

This patch also improves the management of the EXEC_BACKEND
ShmemBackendArray array a bit, by reducing search costs.

Although this problem is of long standing, the lack of field complaints
seems to mean it's not critical enough to risk back-patching; at least
not till we get some more testing of this mechanism.

df7641e25a Add a new GUC variable called "IntervalStyle" that decouples interval output
from DateStyle, and create a new interval style that produces output matching
the SQL standard (at least for interval values that fall within the standard's
restrictions).  IntervalStyle is also used to resolve the conflict between the
standard and traditional Postgres rules for interpreting negative interval
input.

cd00406774 Replace time_t with pg_time_t (same values, but always int64) in on-disk
data structures and backend internal APIs.  This solves problems we've seen
recently with inconsistent layout of pg_control between machines that have
32-bit time_t and those that have already migrated to 64-bit time_t.  Also,
we can get out from under the problem that Windows' Unix-API emulation is not
consistent about the width of time_t.

There are a few remaining places where local time_t variables are used to hold
the current or recent result of time(NULL).  I didn't bother changing these
since they do not affect any cross-module APIs and surely all platforms will
have 64-bit time_t before overflow becomes an actual risk.  time_t should
be avoided for anything visible to extension modules, however.

63872601e8 Move session_start out of MyProcPort stucture and make it a global called MyStartTime,
so that we will be able to create a cookie for all processes for CSVlogs.
It is set wherever MyProcPid is set. Take the opportunity to remove the now
unnecessary session-only restriction on the %s and %c escapes in log_line_prefix.

e2a186b03c Add a multi-worker capability to autovacuum. This allows multiple worker
processes to be running simultaneously.  Also, now autovacuum processes do not
count towards the max_connections limit; they are counted separately from
regular processes, and are limited by the new GUC variable
autovacuum_max_workers.

The launcher now has intelligence to launch workers on each database every
autovacuum_naptime seconds, limited only on the max amount of worker slots
available.

Also, the global worker I/O utilization is limited by the vacuum cost-based
delay feature.  Workers are "balanced" so that the total I/O consumption does
not exceed the established limit.  This part of the patch was contributed by
ITAGAKI Takahiro.

eb5949d190 Arrange for the postmaster (and standalone backends, initdb, etc) to
chdir into PGDATA and subsequently use relative paths instead of absolute
paths to access all files under PGDATA.  This seems to give a small
performance improvement, and it should make the system more robust
against naive DBAs doing things like moving a database directory that
has a live postmaster in it.  Per recent discussion.

2467394ee1 Tablespaces. Alternate database locations are dead, long live tablespaces.
There are various things left to do: contrib dbsize and oid2name modules
need work, and so does the documentation.  Also someone should think about
COMMENT ON TABLESPACE and maybe RENAME TABLESPACE.  Also initlocation is
dead, it just doesn't know it yet.

076a055acf Separate out bgwriter code into a logically separate module, rather
than being random pieces of other files.  Give bgwriter responsibility
for all checkpoint activity (other than a post-recovery checkpoint);
so this child process absorbs the functionality of the former transient
checkpoint and shutdown subprocesses.  While at it, create an actual
include file for postmaster.c, which for some reason never had its own
file before.

1a321f26d8 Code review for EXEC_BACKEND changes. Reduce the number of #ifdefs by
about a third, make it work on non-Windows platforms again.  (But perhaps
I broke the WIN32 code, since I have no way to test that.)  Fold all the
paths that fork postmaster child processes to go through the single
routine SubPostmasterMain, which takes care of resurrecting the state that
would normally be inherited from the postmaster (including GUC variables).
Clean up some places where there's no particularly good reason for the
EXEC and non-EXEC cases to work differently.  Take care of one or two
FIXMEs that remained in the code.

58f337a343 Centralize implementation of delay code by creating a pg_usleep()
subroutine in src/port/pgsleep.c.  Remove platform dependencies from
miscadmin.h and put them in port.h where they belong.  Extend recent
vacuum cost-based-delay patch to apply to VACUUM FULL, ANALYZE, and
non-btree index vacuuming.

By the way, where is the documentation for the cost-based-delay patch?

391c3811a2 Rename SortMem and VacuumMem to work_mem and maintenance_work_mem.
Make btree index creation and initial validation of foreign-key constraints
use maintenance_work_mem rather than work_mem as their memory limit.
Add some code to guc.c to allow these variables to be referenced by their
old names in SHOW and SET commands, for backwards compatibility.

2f0d43b251 Review uses of IsUnderPostmaster, change some tests to look at
whereToSendOutput instead because they are really inquiring about
the correct client communication protocol.  Update some comments.
This is pointing towards supporting regular FE/BE client protocol
in a standalone backend, per discussion a month or so back.

f63cbccac7 Mop-up for previous change to determine default shared_buffers and
max_connections at initdb time.  Get rid of DEF_NBUFFERS and DEF_MAXBACKENDS
macros, which aren't doing anything useful anymore, and put more likely
defaults into postgresql.conf.sample.
