2421e9a51d Remove read/sync fields from pg_stat_wal and GUC track_wal_io_timing
The four following attributes are removed from pg_stat_wal:
* wal_write
* wal_sync
* wal_write_time
* wal_sync_time

a051e71e28 has added an equivalent of this information in pg_stat_io
with more granularity as this now spreads across the backend types, IO
context and IO objects.  So, keeping the same information in pg_stat_wal
has little benefits.

Another benefit of this commit is the removal of PendingWalStats,
simplifying an upcoming patch to add per-backend WAL statistics, which
already support IO statistics and which have access to the write/sync
stats data of WAL.

The GUC track_wal_io_timing, that was used to enable or disable the
aggregation of the write and sync timings for WAL, is also removed.
pgstat_prepare_io_time() is simplified.

Bump catalog version.
Bump PGSTAT_FILE_FORMAT_ID, due to the update of PgStat_WalStats.

827b4060a8 Remove unnecessary (char *) casts [mem]
Remove (char *) casts around memory functions such as memcmp(),
memcpy(), or memset() where the cast is useless.  Since these
functions don't take char * arguments anyway, these casts are at best
complicated casts to (void *), about which see commit 7f798aca1d.

f92c854cf4 Make pg_stat_io count IOs as bytes instead of blocks for some operations
Currently in pg_stat_io view, IOs are counted as blocks of size
BLCKSZ.  There are two limitations with this design:
* The actual number of I/O requests sent to the kernel is lower because
I/O requests may be merged before being sent.  Additionally, it gives
the impression that all I/Os are done in block size, which shadows the
benefits of merging I/O requests.
* Some patches are under work to extend pg_stat_io for the tracking of
operations that may not be linked to the block size.  For example, WAL
read IOs are done in variable bytes and it is not possible to correctly
show these IOs in pg_stat_io view, and we want to keep all this data in
a single system view rather than spread it across multiple relations to
ease monitoring.

WaitReadBuffers() can now be tracked as a single read operation
worth N blocks.  Same for ExtendBufferedRelShared() and
ExtendBufferedRelLocal() for extensions.

Three columns are added to pg_stat_io for reads, writes and extensions
for the byte calculations.  op_bytes, which was always hardcoded to
BLCKSZ, is removed.  IO backend statistics are updated to reflect these
changes.

Bump catalog version.

f0bf7857be Merge pgstat_count_io_op_n() and pgstat_count_io_op()
The pgstat_count_io_op() function, which counts a single I/O operation,
wraps pgstat_count_io_op_n() with a counter value of 1.  The latter is
declared in pgstat.h and used nowhere in the code, so let's remove it in
favor of the former.

This change makes also the code more symmetric with
pgstat_count_io_op_time(), that already uses a similar set of arguments,
except that it counts also the I/O time.  This will ease a bit the
integration of a follow-up patch that adds byte-level tracking in
pg_stat_io for some of its attributes, lifting the current restriction
based on BLCKSZ as all I/O operations are assumed to be block-based.

17974ec259 Revise GUC names quoting in messages again
After further review, we want to move in the direction of always
quoting GUC names in error messages, rather than the previous (PG16)
wildly mixed practice or the intermittent (mid-PG17) idea of doing
this depending on how possibly confusing the GUC name is.

This commit applies appropriate quotes to (almost?) all mentions of
GUC names in error messages.  It partially supersedes a243569bf6 and
8d9978a717, which had moved things a bit in the opposite direction
but which then were abandoned in a partial state.

210622c60e Provide vectored variant of ReadBuffer().
Break ReadBuffer() up into two steps.  StartReadBuffers() and
WaitReadBuffers() give us two main advantages:

1.  Multiple consecutive blocks can be read with one system call.
2.  Advice (hints of future reads) can optionally be issued to the
kernel ahead of time.

The traditional ReadBuffer() function is now implemented in terms of
those functions, to avoid duplication.

A new GUC io_combine_limit is defined, and the functions for limiting
per-backend pin counts are made into public APIs.  Those are provided
for use by callers of StartReadBuffers(), when deciding how many buffers
to read at once.  The following commit will add a higher level mechanism
for doing that automatically with a practical interface.

With some more infrastructure in later work, StartReadBuffers() could
be extended to start real asynchronous I/O instead of just issuing
advice and leaving WaitReadBuffers() to do the work synchronously.

dbbca2cf29 Remove unused #include's from backend .c files
as determined by include-what-you-use (IWYU)

While IWYU also suggests to *add* a bunch of #include's (which is its
main purpose), this patch does not do that.  In some cases, a more
specific #include replaces another less specific one.

Some manual adjustments of the automatic result:

- IWYU currently doesn't know about includes that provide global
  variable declarations (like -Wmissing-variable-declarations), so
  those includes are being kept manually.

- All includes for port(ability) headers are being kept for now, to
  play it safe.

- No changes of catalog/pg_foo.h to catalog/pg_foo_d.h, to keep the
  patch from exploding in size.

Note that this patch touches just *.c files, so nothing declared in
header files changes in hidden ways.

As a small example, in src/backend/access/transam/rmgr.c, some IWYU
pragma annotations are added to handle a special case there.

024c521117 Replace BackendIds with 0-based ProcNumbers
Now that BackendId was just another index into the proc array, it was
redundant with the 0-based proc numbers used in other places. Replace
all usage of backend IDs with proc numbers.

The only place where the term "backend id" remains is in a few pgstat
functions that expose backend IDs at the SQL level. Those IDs are now
in fact 0-based ProcNumbers too, but the documentation still calls
them "backend ids". That term still seems appropriate to describe what
the numbers are, so I let it be.

One user-visible effect is that pg_temp_0 is now a valid temp schema
name, for backend with ProcNumber 0.

d212957254 Fix bug in bulk extending temp relation after failure
A ResourceOwnerEnlarge() call was missing. That led to an error:

ERROR:  ResourceOwnerRemember called but array was full

and an assertion failure, if you tried to extend a temp relation again
after a failure. Alexander's test case used running out of disk space
to trigger the original failure.

This bug was introduced in the large ResourceOwner rewrite commit
b8bff07daa. Before that, the UnpinLocalBuffer() call guaranteed that
the subsequent PinLocalBuffer() will succeed, but after the rewrite,
releasing an old resource doesn't guarantee that there is space for a
new one.

Add a comment explaining why the UnpinBuffer + PinBuffer calls in
BufferAlloc(), with no ResourceOwnerEnlarge() in between, are safe.

9cd0d77dfc Fix corruption of local buffer state during extend of temp relation
A typo has been introduced by 31966b151e when updating the state of a
local buffer when a temporary relation is extended, for the case of a
block included in the relation range extended, when it is already found
in the hash table holding the local buffers.  In this case, BM_VALID
should be cleared, but the buffer state was changed so as BM_VALID
remained while clearing the other flags.

As reported on the thread, it was possible to corrupt the state of the
local buffers on ENOSPC, but the states would be corrupted on any kind
of ERROR during the relation extend (like partial writes or some other
errno).

3c9d9acae0 Refactor pgstat_prepare_io_time() with an input argument instead of a GUC
Originally, this routine relied on track_io_timing to check if a time
interval for an I/O operation stored in pg_stat_io should be initialized
or not.  However, the addition of WAL statistics to pg_stat_io requires
that the initialization happens when track_wal_io_timing is enabled,
which is dependent on the code path where the I/O operation happens.

b485ad7f07 Provide multi-block smgrprefetch().
Previously smgrprefetch() could issue POSIX_FADV_WILLNEED advice for a
single block at a time.  Add an nblocks argument so that we can do the
same for a range of blocks.  This usually produces a single system call,
but might need to loop if it crosses a segment boundary.  Initially it
is only called with nblocks == 1, but proposed patches will make wider
calls.

8d9978a717 Apply quotes more consistently to GUC names in logs
Quotes are applied to GUCs in a very inconsistent way across the code
base, with a mix of double quotes or no quotes used.  This commit
removes double quotes around all the GUC names that are obviously
referred to as parameters with non-English words (use of underscore,
mixed case, etc).

This is the result of a discussion with Álvaro Herrera, Nathan Bossart,
Laurenz Albe, Peter Eisentraut, Tom Lane and Daniel Gustafsson.

b8bff07daa Make ResourceOwners more easily extensible.
Instead of having a separate array/hash for each resource kind, use a
single array and hash to hold all kinds of resources. This makes it
possible to introduce new resource "kinds" without having to modify
the ResourceOwnerData struct. In particular, this makes it possible
for extensions to register custom resource kinds.

The old approach was to have a small array of resources of each kind,
and if it fills up, switch to a hash table. The new approach also uses
an array and a hash, but now the array and the hash are used at the
same time. The array is used to hold the recently added resources, and
when it fills up, they are moved to the hash. This keeps the access to
recent entries fast, even when there are a lot of long-held resources.

All the resource-specific ResourceOwnerEnlarge*(),
ResourceOwnerRemember*(), and ResourceOwnerForget*() functions have
been replaced with three generic functions that take resource kind as
argument. For convenience, we still define resource-specific wrapper
macros around the generic functions with the old names, but they are
now defined in the source files that use those resource kinds.

The release callback no longer needs to call ResourceOwnerForget on
the resource being released. ResourceOwnerRelease unregisters the
resource from the owner before calling the callback. That needed some
changes in bufmgr.c and some other files, where releasing the
resources previously always called ResourceOwnerForget.

Each resource kind specifies a release priority, and
ResourceOwnerReleaseAll releases the resources in priority order. To
make that possible, we have to restrict what you can do between
phases. After calling ResourceOwnerRelease(), you are no longer
allowed to remember any more resources in it or to forget any
previously remembered resources by calling ResourceOwnerForget.  There
was one case where that was done previously. At subtransaction commit,
AtEOSubXact_Inval() would handle the invalidation messages and call
RelationFlushRelation(), which temporarily increased the reference
count on the relation being flushed. We now switch to the parent
subtransaction's resource owner before calling AtEOSubXact_Inval(), so
that there is a valid ResourceOwner to temporarily hold that relcache
reference.

Other end-of-xact routines make similar calls to AtEOXact_Inval()
between release phases, but I didn't see any regression test failures
from those, so I'm not sure if they could reach a codepath that needs
remembering extra resources.

There were two exceptions to how the resource leak WARNINGs on commit
were printed previously: llvmjit silently released the context without
printing the warning, and a leaked buffer io triggered a PANIC. Now
everything prints a WARNING, including those cases.

Add tests in src/test/modules/test_resowner.

b70c2143bb Move a few ResourceOwnerEnlarge() calls for safety and clarity.
These are functions where a lot of things happen between the
ResourceOwnerEnlarge and ResourceOwnerRemember calls. It's important
that there are no unrelated ResourceOwnerRemember calls in the code in
between, otherwise the reserved entry might be used up by the
intervening ResourceOwnerRemember and not be available at the intended
ResourceOwnerRemember call anymore. I don't see any bugs here, but the
longer the code path between the calls is, the harder it is to verify.

In bufmgr.c, there is a function similar to ResourceOwnerEnlarge,
ReservePrivateRefCountEntry(), to ensure that the private refcount
array has enough space. The ReservePrivateRefCountEntry() calls were
made at different places than the ResourceOwnerEnlargeBuffers()
calls. Move the ResourceOwnerEnlargeBuffers() and
ReservePrivateRefCountEntry() calls together for consistency.

7369798a83 Fix tracking of temp table relation extensions as writes
Karina figured out that I (Andres) confused BufferUsage.temp_blks_written with
BufferUsage.local_blks_written in fcdda1e4b5.

Tests in core PG can't easily test this, as BufferUsage is just used for
EXPLAIN (ANALYZE, BUFFERS) and pg_stat_statements. Thus this commit adds tests
for this to pg_stat_statements.

7114791158 ExtendBufferedWhat -> BufferManagerRelation.
Commit 31966b15 invented a way for functions dealing with relation
extension to accept a Relation in online code and an SMgrRelation in
recovery code.  It seems highly likely that future bufmgr.c interfaces
will face the same problem, and need to do something similar.
Generalize the names so that each interface doesn't have to re-invent
the wheel.

Back-patch to 16.  Since extension AM authors might start using the
constructor macros once 16 ships, we agreed to do the rename in 16
rather than waiting for 17.

d4e71df6d7 Add io_direct setting (developer-only).
Provide a way to ask the kernel to use O_DIRECT (or local equivalent)
where available for data and WAL files, to avoid or minimize kernel
caching.  This hurts performance currently and is not intended for end
users yet.  Later proposed work would introduce our own I/O clustering,
read-ahead, etc to replace the facilities the kernel disables with this
option.

The only user-visible change, if the developer-only GUC is not used, is
that this commit also removes the obscure logic that would activate
O_DIRECT for the WAL when wal_sync_method=open_[data]sync and
wal_level=minimal (which also requires max_wal_senders=0).  Those are
non-default and unlikely settings, and this behavior wasn't (correctly)
documented.  The same effect can be achieved with io_direct=wal.

faeedbcefd Introduce PG_IO_ALIGN_SIZE and align all I/O buffers.
In order to have the option to use O_DIRECT/FILE_FLAG_NO_BUFFERING in a
later commit, we need the addresses of user space buffers to be well
aligned.  The exact requirements vary by OS and file system (typically
sectors and/or memory pages).  The address alignment size is set to
4096, which is enough for currently known systems: it matches modern
sectors and common memory page size.  There is no standard governing
O_DIRECT's requirements so we might eventually have to reconsider this
with more information from the field or future systems.

Aligning I/O buffers on memory pages is also known to improve regular
buffered I/O performance.

Three classes of I/O buffers for regular data pages are adjusted:
(1) Heap buffers are now allocated with the new palloc_aligned() or
MemoryContextAllocAligned() functions introduced by commit 439f6175.
(2) Stack buffers now use a new struct PGIOAlignedBlock to respect
PG_IO_ALIGN_SIZE, if possible with this compiler.  (3) The buffer
pool is also aligned in shared memory.

WAL buffers were already aligned on XLOG_BLCKSZ.  It's possible for
XLOG_BLCKSZ to be configured smaller than PG_IO_ALIGNED_SIZE and thus
for O_DIRECT WAL writes to fail to be well aligned, but that's a
pre-existing condition and will be addressed by a later commit.

BufFiles are not yet addressed (there's no current plan to use O_DIRECT
for those, but they could potentially get some incidental speedup even
in plain buffered I/O operations through better alignment).

If we can't align stack objects suitably using the compiler extensions
we know about, we disable the use of O_DIRECT by setting PG_O_DIRECT to
0.  This avoids the need to consider systems that have O_DIRECT but
can't align stack objects the way we want; such systems could in theory
be supported with more work but we don't currently know of any such
machines, so it's easier to pretend there is no O_DIRECT support
instead.  That's an existing and tested class of system.

Add assertions that all buffers passed into smgrread(), smgrwrite() and
smgrextend() are correctly aligned, unless PG_O_DIRECT is 0 (= stack
alignment tricks may be unavailable) or the block size has been set too
small to allow arrays of buffers to be all aligned.

ac8d53dae5 Track IO times in pg_stat_io
a9c70b46db and 8aaa04b32S added counting of IO operations to a new view,
pg_stat_io. Now, add IO timing for reads, writes, extends, and fsyncs to
pg_stat_io as well.

This combines the tracking for pgBufferUsage with the tracking for pg_stat_io
into a new function pgstat_count_io_op_time(). This should make it a bit
easier to avoid the somewhat costly instr_time conversion done for
pgBufferUsage.

704261ecc6 Improve IO accounting for temp relation writes
Both pgstat_database and pgBufferUsage count IO timing for reads of temporary
relation blocks into local buffers. However, both failed to count write IO
timing for flushes of dirty local buffers. Fix.

Additionally, FlushRelationBuffers() seems to have omitted counting write
IO (both count and timing) stats for both pgstat_database and
pgBufferUsage. Fix.

The primary bottlenecks for relation extension are:

1) The extension lock is held while acquiring a victim buffer for the new
   page. Acquiring a victim buffer can require writing out the old page
   contents including possibly needing to flush WAL.

2) When extending via ReadBuffer() et al, we write a zero page during the
   extension, and then later write out the actual page contents. This can
   nearly double the write rate.

3) The existing bulk relation extension infrastructure in hio.c just amortized
   the cost of acquiring the relation extension lock, but none of the other
   costs.

Unfortunately 1) cannot currently be addressed in a central manner as the
callers to ReadBuffer() need to acquire the extension lock. To address that,
this this commit moves the responsibility for acquiring the extension lock
into bufmgr.c functions. That allows to acquire the relation extension lock
for just the required time. This will also allow us to improve relation
extension further, without changing callers.

The reason we write all-zeroes pages during relation extension is that we hope
to get ENOSPC errors earlier that way (largely works, except for CoW
filesystems). It is easier to handle out-of-space errors gracefully if the
page doesn't yet contain actual tuples. This commit addresses 2), by using the
recently introduced smgrzeroextend(), which extends the relation, without
dirtying the kernel page cache for all the extended pages.

To address 3), this commit introduces a function to extend a relation by
multiple blocks at a time.

There are three new exposed functions: ExtendBufferedRel() for extending the
relation by a single block, ExtendBufferedRelBy() to extend a relation by
multiple blocks at once, and ExtendBufferedRelTo() for extending a relation up
to a certain size.

To avoid duplicating code between ReadBuffer(P_NEW) and the new functions,
ReadBuffer(P_NEW) now implements relation extension with
ExtendBufferedRel(), using a flag to tell ExtendBufferedRel() that the
relation lock is already held.

Note that this commit does not yet lead to a meaningful performance or
scalability improvement - for that uses of ReadBuffer(P_NEW) will need to be
converted to ExtendBuffered*(), which will be done in subsequent commits.

dad50f677c bufmgr: Acquire and clean victim buffer separately
Previously we held buffer locks for two buffer mapping partitions at the same
time to change the identity of buffers.  Particularly for extending relations
needing to hold the extension lock while acquiring a victim buffer is
painful.But it also creates a bottleneck for workloads that just involve
reads.

Now we instead first acquire a victim buffer and write it out, if
necessary. Then we remove that buffer from the old partition with just the old
partition's partition lock held and insert it into the new partition with just
that partition's lock held.

By separating out the victim buffer acquisition, future commits will be able
to change relation extensions to scale better.

On my workstation, a micro-benchmark exercising buffered reads strenuously and
under a lot of concurrency, sees a >2x improvement.

8aaa04b32d Track shared buffer hits in pg_stat_io
Among other things, this should make it easier to calculate a useful cache hit
ratio by excluding buffer reads via buffer access strategies. As buffer access
strategies reuse buffers (and thus evict the prior buffer contents), it is
normal to see reads on repeated scans of the same data.

558cf80387 bufmgr: Fix undefined behaviour with, unrealistically, large temp_buffers
Quoting Melanie:

All other places were already using the correct version. I (Andres), copied
the code into more places in a patch. Melanie caught it in review, but to
prevent more people from copying the bad code, fix it. Even if it is a
theoretical issue.

We really ought to wrap these accesses in a helper function...

As this is a theoretical issue, don't backpatch.

f30d62c2fc pgstat: Track more detailed relation IO statistics
Commit 28e626bde0 introduced the infrastructure for tracking more detailed IO
statistics. This commit adds the actual collection of the new IO statistics
for relations and temporary relations. See aforementioned commit for goals and
high-level design.

The changes in this commit are fairly straight-forward. The bulk of the change
is to passing sufficient information to the callsites of pgstat_count_io_op().

A somewhat unsightly detail is that it currently is hard to find a better
place to count fsyncs than in md.c, whereas the other pgstat_count_io_op()
calls are in bufmgr.c/localbuf.c. As the number of fsyncs is tied to md.c
implementation details, it's not obvious there is a better answer.

0a20ff54f5 Split up guc.c for better build speed and ease of maintenance.
guc.c has grown to be one of our largest .c files, making it
a bottleneck for compilation.  It's also acquired a bunch of
knowledge that'd be better kept elsewhere, because of our not
very good habit of putting variable-specific check hooks here.
Hence, split it up along these lines:

* guc.c itself retains just the core GUC housekeeping mechanisms.
* New file guc_funcs.c contains the SET/SHOW interfaces and some
  SQL-accessible functions for GUC manipulation.
* New file guc_tables.c contains the data arrays that define the
  built-in GUC variables, along with some already-exported constant
  tables.
* GUC check/assign/show hook functions are moved to the variable's
  home module, whenever that's clearly identifiable.  A few hard-
  to-classify hooks ended up in commands/variable.c, which was
  already a home for miscellaneous GUC hook functions.

To avoid cluttering a lot more header files with #include "guc.h",
I also invented a new header file utils/guc_hooks.h and put all
the GUC hook functions' declarations there, regardless of their
originating module.  That allowed removal of #include "guc.h"
from some existing headers.  The fallout from that (hopefully
all caught here) demonstrates clearly why such inclusions are
best minimized: there are a lot of files that, for example,
were getting array.h at two or more levels of remove, despite
not having any connection at all to GUCs in themselves.

There is some very minor code beautification here, such as
renaming a couple of inconsistently-named hook functions
and improving some comments.  But mostly this just moves
code from point A to point B and deals with the ensuing
needs for #include adjustments and exporting a few functions
that previously weren't exported.

Patch by me, per a suggestion from Andres Freund; thanks also
to Michael Paquier for the idea to invent guc_funcs.c.

82ac34db20 Include RelFileLocator fields individually in BufferTag.
This is preparatory work for a project to increase the number of bits
in a RelFileNumber from 32 to 56.

Along the way, introduce static inline accessor functions for a couple
of BufferTag fields.

b0a55e4329 Change internal RelFileNode references to RelFileNumber or RelFileLocator.
We have been using the term RelFileNode to refer to either (1) the
integer that is used to name the sequence of files for a certain relation
within the directory set aside for that tablespace/database combination;
or (2) that value plus the OIDs of the tablespace and database; or
occasionally (3) the whole series of files created for a relation
based on those values. Using the same name for more than one thing is
confusing.

Replace RelFileNode with RelFileNumber when we're talking about just the
single number, i.e. (1) from above, and with RelFileLocator when we're
talking about all the things that are needed to locate a relation's files
on disk, i.e. (2) from above. In the places where we refer to (3) as
a relfilenode, instead refer to "relation storage".

Since there is a ton of SQL code in the world that knows about
pg_class.relfilenode, don't change the name of that column, or of other
SQL-facing things that derive their name from it.

On the other hand, do adjust closely-related internal terminology. For
example, the structure member names dbNode and spcNode appear to be
derived from the fact that the structure itself was called RelFileNode,
so change those to dbOid and spcOid. Likewise, various variables with
names like rnode and relnode get renamed appropriately, according to
how they're being used in context.

Hopefully, this is clearer than before. It is also preparation for
future patches that intend to widen the relfilenumber fields from its
current width of 32 bits. Variables that store a relfilenumber are now
declared as type RelFileNumber rather than type Oid; right now, these
are the same, but that can now more easily be changed.

b3817f5f77 Improve hash_create()'s API for some added robustness.
Invent a new flag bit HASH_STRINGS to specify C-string hashing, which
was formerly the default; and add assertions insisting that exactly
one of the bits HASH_STRINGS, HASH_BLOBS, and HASH_FUNCTION be set.
This is in hopes of preventing recurrences of the type of oversight
fixed in commit a1b8aa1e4 (i.e., mistakenly omitting HASH_BLOBS).

Also, when HASH_STRINGS is specified, insist that the keysize be
more than 8 bytes.  This is a heuristic, but it should catch
accidental use of HASH_STRINGS for integer or pointer keys.
(Nearly all existing use-cases set the keysize to NAMEDATALEN or
more, so there's little reason to think this restriction should
be problematic.)

Tweak hash_create() to insist that the HASH_ELEM flag be set, and
remove the defaults it had for keysize and entrysize.  Since those
defaults were undocumented and basically useless, no callers
omitted HASH_ELEM anyway.

Also, remove memset's zeroing the HASHCTL parameter struct from
those callers that had one.  This has never been really necessary,
and while it wasn't a bad coding convention it was confusing that
some callers did it and some did not.  We might as well save a few
cycles by standardizing on "not".

Also improve the documentation for hash_create().

In passing, improve reinit.c's usage of a hash table by storing
the key as a binary Oid rather than a string; and, since that's
a temporary hash table, allocate it in CurrentMemoryContext for
neatness.

3985b600f5 Support PrefetchBuffer() in recovery.
Provide PrefetchSharedBuffer(), a variant that takes SMgrRelation, for
use in recovery.  Rename LocalPrefetchBuffer() to PrefetchLocalBuffer()
for consistency.

Add a return value to all of these.  In recovery, tolerate and report
missing files, so we can handle relations unlinked before crash recovery
began.  Also report cache hits and misses, so that callers can do faster
buffer lookups and better I/O accounting.

2eb4a831e5 Change TRUE/FALSE to true/false
The lower case spellings are C and C++ standard and are used in most
parts of the PostgreSQL sources.  The upper case spellings are only used
in some files/modules.  So standardize on the standard spellings.

The APIs for ICU, Perl, and Windows define their own TRUE and FALSE, so
those are left as is when using those APIs.

In code comments, we use the lower-case spelling for the C concepts and
keep the upper-case spelling for the SQL concepts.

382ceffdf7 Phase 3 of pgindent updates.
Don't move parenthesized lines to the left, even if that means they
flow past the right margin.

By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis.  However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent.  That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.

This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.

This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.
