GIN improvements

Browse Source 
- Replace sorted array of entries in maintenance_work_mem to binary tree,
  this should improve create performance.
- More precisely calculate allocated memory, eliminate leaks
  with user-defined extractValue()
- Improve wordings in tsearch2
