  Fix for bug #46044 "MDL deadlock on LOCK TABLE + CREATE TABLE HIGH_PRIORITY
  FOR UPDATE".

  Deadlock occured when during execution of query to I_S we tried to open
  a table or its .FRM in order to get information about it and had to wait
  because we have encountered exclusive metadata lock on this table held by
  a DDL operation from another connection which in its turn waited for some
  resource currently owned by connection executing this I_S query.
  For example, this might have happened if one under LOCK TABLES executed I_S
  query targeted to particular table (which was not among locked) and also
  concurrently tried to create this table using CREATE TABLE SELECT which
  had to wait for one of tables locked by the first connection.
  Another situation in which deadlock might have occured is when I_S query,
  which was executed as part of transaction, tried to get information about
  table which just has been dropped by concurrent DROP TABLES executed under
  LOCK TABLES and this DROP TABLES for its completion also had to wait
  transaction from the first connection.

  This problem stemmed from the fact that opening of tables/.FRMs for I_S
  filling is happening outside of connection's main MDL_context so code
  which tries to detect deadlocks due to conflicting metadata locks doesn't
  work in this case. Indeed, this led to deadlocks when during I_S filling
  we tried to wait for conflicting metadata lock to go away, while its owner
  was waiting for some resource held by connection executing I_S query.

  This patch solves this problem by avoiding waiting in such situation.
  Instead we skip this table and produce warning that information about
  it was omitted from I_S due to concurrent DDL operation. We still wait
  for conflicting metadata lock to go away when it is known that deadlock
  is not possible (i.e. when connection executing I_S query does not hold
  any metadata or table-level locks).
  Basically, we apply our standard deadlock avoidance technique for metadata
  locks to the process of filling of I_S tables but replace ER_LOCK_DEADLOCK
  error with a warning.
  Note that this change is supposed to be safe for 'mysqldump' since the
  only its mode which is affected by this change is --single-transaction mode
  is not safe in the presence of concurrent DDL anyway (and this fact is
  documented). Other modes are unaffected because they either use
  SHOW TABLES/SELECT * FROM I_S.TABLE_NAMES which do not take any metadata
  locks in the process of I_S table filling and thus cannot skip tables or
  execute I_S queries for tables which were previously locked by LOCK TABLES
  (or in the presence of global read lock) which excludes possibility of
  encountering conflicting metadata lock.

  Bug#36785: Wrong error message when group_concat() exceeds max length

  The problem is that when ER_CUT_VALUE_GROUP_CONCAT is elevated
  to a error, the message does not get updated with the number of
  cut lines when group_concat() exceeds max length.

  The solution is to modify the warning message to be more meaningful
  by giving the number of the line that was cut and to issue the warning
  for each line that is cut. This approach is inline with how other
  per-row truncated data warnings are issued avoids violating the warning
  internal interface.

Non-transactional updates that take place inside a transaction present problems
for logging because they are visible to other clients before the transaction
is committed, and they are not rolled back even if the transaction is rolled
back. It is not always possible to log correctly in statement format when both
transactional and non-transactional tables are used in the same transaction.

In the current patch, we ensure that such scenario is completely safe under the
ROW and MIXED modes.

2d6a356abf Bug#38551 query cache can still consume [very little] cpu time even when it is off.
When the query cache is disabled, the server shouldn't attempt to take the 
query cache mutex.

73b296c4c4 BUG#39934: Slave stops for engine that only support row-based logging
Post-push fix.
Problem: After the original bugfix, if a statement is unsafe,
binlog_format=mixed, and engine is statement-only, a warning was
generated and the statement executed. However, it is a fundamental
principle of binlogging that binlog_format=mixed should guarantee
correct logging, no compromise. So correct behavior is to generate
an error and don't execute the statement.
Fix: Generate error instead of warning.
Since issue_unsafe_warnings can only generate one error message,
this allows us to simplify the code a bit too:
decide_logging_format does not have to save the error code for
issue_unsafe_warnings

Backport from 6.0 to 5.1.
Only those sync points are included, which are used in debug_sync.test.

  The Debug Sync Facility allows to place synchronization points
  in the code:
  
  open_tables(...)
  
  DEBUG_SYNC(thd, "after_open_tables");
  
  lock_tables(...)
  
  When activated, a sync point can
  
  - Send a signal and/or
  - Wait for a signal
  
  Nomenclature:
  
  - signal:            A value of a global variable that persists
                       until overwritten by a new signal. The global
                       variable can also be seen as a "signal post"
                       or "flag mast". Then the signal is what is
                       attached to the "signal post" or "flag mast".
  
  - send a signal:     Assign the value (the signal) to the global
                       variable ("set a flag") and broadcast a
                       global condition to wake those waiting for
                       a signal.
  
  - wait for a signal: Loop over waiting for the global condition until
                       the global value matches the wait-for signal.

7079338e0e Bug #42914 Log event that larger than max_allowed_packet results in stop of slave I/O thread,
But there is no Last_IO_Error reported.

On the master, if a binary log event is larger than max_allowed_packet,
ER_MASTER_FATAL_ERROR_READING_BINLOG and the specific reason of this error is
sent to a slave when it requests a dump from the master, thus leading
the I/O thread to stop.

On a slave, the I/O thread stops when receiving a packet larger than max_allowed_packet.

In both cases, however, there was no Last_IO_Error reported.

This patch adds code to report the Last_IO_Error and exact reason before stopping the
I/O thread and also reports the case the out memory pops up while
handling packets from the master.

4a3876c3df Bug#32430: 'show innodb status' causes errors
Invalid (old?) table or database name in logs

Post push patch.

Bug was that a non partitioned table file was not
converted to system_charset, (due to table_name_len was not set).

Also missing DBUG_RETURN.

And Innodb adds quotes after calling the function,
so I added one more mode where explain_filename does not
add quotes. But it still appends the [sub]partition name
as a comment.

Also caught a minor quoting bug, the character '`' was
not quoted in the identifier. (so 'a`b' was quoted as `a`b`
and not `a``b`, this is mulitbyte characters aware.)

e642140b18 Bug# 30946: mysqldump silently ignores --default-character-set
when used with --tab

1) New syntax: added CHARACTER SET clause to the
  SELECT ... INTO OUTFILE (to complement the same clause in
  LOAD DATA INFILE).
  mysqldump is updated to use this in --tab mode.

2) ESCAPED BY/ENCLOSED BY field parameters are documented as
   accepting CHAR argument, however SELECT .. INTO OUTFILE
   silently ignored rests of multisymbol arguments.
   For the symmetrical behavior with LOAD DATA INFILE the
   server has been modified to fail with the same error:

     ERROR 42000: Field separator argument is not what is
                  expected; check the manual

3) Current LOAD DATA INFILE recognizes field/line separators
   "as is" without converting from client charset to data
   file charset. So, it is supposed, that input file of
   LOAD DATA INFILE consists of data in one charset and
   separators in other charset. For the compatibility with
   that [buggy] behaviour SELECT INTO OUTFILE implementation
   has been saved "as is" too, but the new warning message
   has been added:

     Non-ASCII separator arguments are not fully supported

   This message warns on field/line separators that contain
   non-ASCII symbols.

cbce1f5e69 Fix for BUG#18828 - If InnoDB runs out of undo slots,
it returns misleading 'table is full'

Innodb returns a misleading error message "table is full" 
when the number of active concurrent transactions is greater
than 1024.

Fixed by adding errorcode "ER_TOO_MANY_CONCURRENT_TRXS" to the
error codes. Innodb should return HA_TOO_MANY_CONCURRENT_TRXS
to mysql which is then mapped to ER_TOO_MANY_CONCURRENT_TRXS


Note: testcase is not written as this was reproducible only by
      changing innodb code.

f3985c649d BUG#39934: Slave stops for engine that only support row-based logging
General overview:
The logic for switching to row format when binlog_format=MIXED had
numerous flaws. The underlying problem was the lack of a consistent
architecture.
General purpose of this changeset:
This changeset introduces an architecture for switching to row format
when binlog_format=MIXED. It enforces the architecture where it has
to. It leaves some bugs to be fixed later. It adds extensive tests to
verify that unsafe statements work as expected and that appropriate
errors are produced by problems with the selection of binlog format.
It was not practical to split this into smaller pieces of work.

Problem 1:
To determine the logging mode, the code has to take several parameters
into account (namely: (1) the value of binlog_format; (2) the
capabilities of the engines; (3) the type of the current statement:
normal, unsafe, or row injection). These parameters may conflict in
several ways, namely:
 - binlog_format=STATEMENT for a row injection
 - binlog_format=STATEMENT for an unsafe statement
 - binlog_format=STATEMENT for an engine only supporting row logging
 - binlog_format=ROW for an engine only supporting statement logging
 - statement is unsafe and engine does not support row logging
 - row injection in a table that does not support statement logging
 - statement modifies one table that does not support row logging and
   one that does not support statement logging
Several of these conflicts were not detected, or were detected with
an inappropriate error message. The problem of BUG#39934 was that no
appropriate error message was written for the case when an engine
only supporting row logging executed a row injection with
binlog_format=ROW. However, all above cases must be handled.
Fix 1:
Introduce new error codes (sql/share/errmsg.txt). Ensure that all
conditions are detected and handled in decide_logging_format()

Problem 2:
The binlog format shall be determined once per statement, in
decide_logging_format(). It shall not be changed before or after that.
Before decide_logging_format() is called, all information necessary to
determine the logging format must be available. This principle ensures
that all unsafe statements are handled in a consistent way.
However, this principle is not followed:
thd->set_current_stmt_binlog_row_based_if_mixed() is called in several
places, including from code executing UPDATE..LIMIT,
INSERT..SELECT..LIMIT, DELETE..LIMIT, INSERT DELAYED, and
SET @@binlog_format. After Problem 1 was fixed, that caused
inconsistencies where these unsafe statements would not print the
appropriate warnings or errors for some of the conflicts.
Fix 2:
Remove calls to THD::set_current_stmt_binlog_row_based_if_mixed() from
code executed after decide_logging_format(). Compensate by calling the
set_current_stmt_unsafe() at parse time. This way, all unsafe statements
are detected by decide_logging_format().

Problem 3:
INSERT DELAYED is not unsafe: it is logged in statement format even if
binlog_format=MIXED, and no warning is printed even if
binlog_format=STATEMENT. This is BUG#45825.
Fix 3:
Made INSERT DELAYED set itself to unsafe at parse time. This allows
decide_logging_format() to detect that a warning should be printed or
the binlog_format changed.

Problem 4:
LIMIT clause were not marked as unsafe when executed inside stored
functions/triggers/views/prepared statements. This is
BUG#45785.
Fix 4:
Make statements containing the LIMIT clause marked as unsafe at
parse time, instead of at execution time. This allows propagating
unsafe-ness to the view.

Problem was that InnoDB used filenam_to_tablename,
which do not handle partitions (due to the '#' in
the filename).

Solution is to add a new function for explaining
what the filename means: explain_filename.
It expands the database, table, partition and subpartition
parts and uses errmsg.txt for localization.
It also converts from my_charset_filename to system_charset_info
(i.e. human readable form for non ascii characters).
