04c5e5211e Bug#13105873 :Valgrind Warning: CRASH IN FOREIGN
KEY HANDLING ON SUBSEQUENT CREATE TABLE IF NOT EXISTS
      
      PROBLEM:
      --------
      Consider a SP routine which does CREATE TABLE
      with REFERENCES clause. The first call to this routine
      invokes parser and the parsed items are cached, so as 
      to avoid parsing for the second execution of the routine.
      
      It is obsevered that valgrind reports a warning
      upon read of thd->lex->alter_info->key_list->Foreign_key object,
      which seem to be pointing to a invalid memory address
      during second time execution of the routine. Accessing this object
      theoretically could cause a crash.
      
      ANALYSIS:
      ---------
      The problem stems from the fact that for some reason
      elements of ref_columns list in thd->lex->alter_info->
      key_list->Foreign_key object are changed to point to
      objects allocated on runtime memory root.
      
      During the first execution of routine we create
      a copy of thd->lex->alter_info object.
      As part of this process we create a clones of objects in
      Alter_info::key_list and of Foreign_key object in particular.
      Then Foreign_key object is cloned for some reason we
      perform shallow copies of both Foreign_key::ref_columns
      and Foreign_key::columns list. So new instance of 
      Foreign_key object starts to SHARE contents of ref_columns
      and columns list with the original instance.
      After that as part of cloning process we call
      list_copy_and_replace_each_value() for elements of
      ref_columns list. As result ref_columns lists in both
      original and cloned Foreign_key object start to contain
      pointers to Key_part_spec objects allocated on runtime
      memory root because of shallow copy.
      
      So when we start copying of thd->lex->alter_info object
      during the second execution of stored routine we indeed
      encounter pointer to the Key_part_spec object allocated
      on runtime mem-root which was cleared during at the end
      of previous execution. This is done in sp_head::execute(), 
      by a call to free_root(&execute_mem_root,MYF(0));
      As result we get valgrind warnings about accessing 
      unreferenced memory.
      
      FIX:
      ----
      The safest solution to this problem is to 
      fix Foreign_key(Foreign_key, MEM_ROOT) constructor to do
      a deep copy of columns lists, similar to Key(Key, MEM_ROOT) 
      constructor.

de3693a1cd Bug#11766594 59736: SELECT DISTINCT.. INCORRECT RESULT WITH DETERMINISTIC FUNCTION IN WHERE C
There is an optimization of DISTINCT in JOIN::optimize()
which depends on THD::used_tables value. Each SELECT statement
inside SP resets used_tables value(see mysql_select()) and it
leads to wrong result. The fix is to replace THD::used_tables
with LEX::used_tables.

55d3381cc7 Bug#11764633 : 57491: THD->MAIN_DA.IS_OK() ASSERT IN EMBEDDED
Issue:
While running embedded server, if client issues TEE command (\T foo/bar) and
"foo/bar" directory doesn't exist, it is suppose to give error. But it was
aborting.  This was happening because wrong error handler was being called.

Solution:
Modified calls to correct error handler. In embedded server case, there are 
two error handler (client and server) which are supposed to be called based 
on which context code is in. If it is in client context, client error handler
should be called otherwise server.

Test case:
Test case automation is not possible as current (following) code doesn't 
allow '\T' to be executed from command line (OR command read from a file):

25221cccd2 Fix for BUG#11755168 '46895: test "outfile_loaddata" fails (reproducible)'.
In sql_class.cc, 'row_count', of type 'ha_rows', was used as last argument for
ER_TRUNCATED_WRONG_VALUE_FOR_FIELD which is
"Incorrect %-.32s value: '%-.128s' for column '%.192s' at row %ld".
So 'ha_rows' was used as 'long'.
On SPARC32 Solaris builds, 'long' is 4 bytes and 'ha_rows' is 'longlong' i.e. 8 bytes.
So the printf-like code was reading only the first 4 bytes.
Because the CPU is big-endian, 1LL is 0x00 0x00 0x00 0x00 0x00 0x00 0x00 0x01
so the first four bytes yield 0. So the warning message had "row 0" instead of
"row 1" in test outfile_loaddata.test:
-Warning	1366	Incorrect string value: '\xE1\xE2\xF7' for column 'b' at row 1
+Warning	1366	Incorrect string value: '\xE1\xE2\xF7' for column 'b' at row 0

All error-messaging functions which internally invoke some printf-life function
are potential candidate for such mistakes.
One apparently easy way to catch such mistakes is to use
ATTRIBUTE_FORMAT (from my_attribute.h).
But this works only when call site has both:
a) the format as a string literal
b) the types of arguments.
So:
  func(ER(ER_BLAH), 10);
will silently not be checked, because ER(ER_BLAH) is not known at
compile time (it is known at run-time, and depends on the chosen
language).
And
  func("%s", a va_list argument);
has the same problem, as the *real* type of arguments is not
known at this site at compile time (it's known in some caller).
Moreover,
  func(ER(ER_BLAH));
though possibly correct (if ER(ER_BLAH) has no '%' markers), will not
compile (gcc says "error: format not a string literal and no format
arguments").

Consequences:
1) ATTRIBUTE_FORMAT is here added only to functions which in practice
take "string literal" formats: "my_error_reporter" and "print_admin_msg".
2) it cannot be added to the other functions: my_error(),
push_warning_printf(), Table_check_intact::report_error(),
general_log_print().

To do a one-time check of functions listed in (2), the following
"static code analysis" has been done:
1) replace
  my_error(ER_xxx, arguments for substitution in format)
with the equivalent
  my_printf_error(ER_xxx,ER(ER_xxx), arguments for substitution in
format),
so that we have ER(ER_xxx) and the arguments *in the same call site*
2) add ATTRIBUTE_FORMAT to push_warning_printf(),
Table_check_intact::report_error(), general_log_print()
3) replace ER(xxx) with the hard-coded English text found in
errmsg.txt (like: ER(ER_UNKNOWN_ERROR) is replaced with
"Unknown error"), so that a call site has the format as string literal
4) this way, ATTRIBUTE_FORMAT can effectively do its job
5) compile, fix errors detected by ATTRIBUTE_FORMAT
6) revert steps 1-2-3.
The present patch has no compiler error when submitted again to the
static code analysis above.
It cannot catch all problems though: see Field::set_warning(), in
which a call to push_warning_printf() has a variable error
(thus, not replacable by a string literal); I checked set_warning() calls
by hand though.

See also WL 5883 for one proposal to avoid such bugs from appearing
again in the future.

The issues fixed in the patch are:
a) mismatch in types (like 'int' passed to '%ld')
b) more arguments passed than specified in the format.
This patch resolves mismatches by changing the type/number of arguments,
not by changing error messages of sql/share/errmsg.txt. The latter would be wrong,
per the following old rule: errmsg.txt must be as stable as possible; no insertions
or deletions of messages, no changes of type or number of printf-like format specifiers,
are allowed, as long as the change impacts a message already released in a GA version.
If this rule is not followed:
- Connectors, which use error message numbers, will be confused (by insertions/deletions
of messages)
- using errmsg.sys of MySQL 5.1.n with mysqld of MySQL 5.1.(n+1)
could produce wrong messages or crash; such usage can easily happen if
installing 5.1.(n+1) while /etc/my.cnf still has --language=/path/to/5.1.n/xxx;
or if copying mysqld from 5.1.(n+1) into a 5.1.n installation.
When fixing b), I have verified that the superfluous arguments were not used in the format
in the first 5.1 GA (5.1.30 'bteam@astra04-20081114162938-z8mctjp6st27uobm').
Had they been used, then passing them today, even if the message doesn't use them
anymore, would have been necessary, as explained above.

1a0dde9206 Bug #11766513 - 59641: Prepared XA transaction in system after hard crash
causes future shutdown hang

InnoDB would hang on shutdown if any XA transactions exist in the
system in the PREPARED state. This has been masked by the fact that
MySQL would roll back any PREPARED transaction on shutdown, in the
spirit of Bug #12161 Xa recovery and client disconnection.

[mysql-test-run] do_shutdown_server: Interpret --shutdown_server 0 as
a request to kill the server immediately without initiating a
shutdown procedure.

xid_cache_insert(): Initialize XID_STATE::rm_error in order to avoid a
bogus error message on XA ROLLBACK of a recovered PREPARED transaction.

innobase_commit_by_xid(), innobase_rollback_by_xid(): Free the InnoDB
transaction object after rolling back a PREPARED transaction.

trx_get_trx_by_xid(): Only consider transactions whose
trx->is_prepared flag is set. The MySQL layer seems to prevent
attempts to roll back connected transactions that are in the PREPARED
state from another connection, but it is better to play it safe. The
is_prepared flag was introduced in the InnoDB Plugin.

trx_n_prepared: A new counter, counting the number of InnoDB
transactions in the PREPARED state.

logs_empty_and_mark_files_at_shutdown(): On shutdown, allow
trx_n_prepared transactions to exist in the system.

trx_undo_free_prepared(), trx_free_prepared(): New functions, to free
the memory objects of PREPARED transactions on shutdown. This is not
needed in the built-in InnoDB, because it would collect all allocated
memory on shutdown. The InnoDB Plugin needs this because of
innodb_use_sys_malloc.

trx_sys_close(): Invoke trx_free_prepared() on all remaining
transactions.

180e74bd49 Bug#27606 GRANT statement should be replicated with DEFINER information
"Grantor" columns' data is lost when replicating mysql.tables_priv.
Slave SQL thread used its default user ''@'' as the grantor of GRANT|REVOKE
statements executing on it.

In this patch, current user is put in query log event for all GRANT and REVOKE
statement, SQL thread uses the user in query log event as grantor.

24fc7ca4c8 Bug#39932 "create table fails if column for FK is in different
case than in corr index".
      
Server was unable to find existing or explicitly created supporting
index for foreign key if corresponding statement clause used field
names in case different than one used in key specification and created
yet another supporting index.
In cases when name of constraint (and thus name of generated index)
was the same as name of existing/explicitly created index this led
to duplicate key name error.
      
The problem was that unlike all other code Key_part_spec::operator==()
compared field names in case sensitive fashion. As result routines
responsible for getting rid of redundant generated supporting indexes
for foreign key were not working properly for versions of field names
using different cases.

16a7308825 Bug #53493 : add_to_status does not handle the longlong fields in STATUS_VAR
bytes_received/bytes_sent are ulonglong so they cannot be handled by the 
ulong handling code in add_to_status/add_diff_to_status().

Fixed by adding code to handle these two variables in 
add_to_status()/add_diff_to_status() and making sure they are not a subject
to the ulong handling code.

c4021e2d43 Bug #53088: mysqldump with -T & --default-character-set set
truncates text/blob to 766 chars

mysqldump and SELECT ... INTO OUTFILE truncated long BLOB/TEXT
values to size of 766 bytes (MAX_FIELD_WIDTH or 255 * 3 + 1).

The select_export::send_data method has been modified to
reallocate a conversion buffer for long field data.

5b6ebdf086 Bug#50373 --secure-file-priv=""
Iterative patch improvement. Previously committed patch
caused wrong result on Windows. The previous patch also
broke secure_file_priv for symlinks since not all file
paths which must be compared against this variable are
normalized using the same norm.

The server variable opt_secure_file_priv wasn't
normalized properly and caused the operations
LOAD DATA INFILE .. INTO TABLE ..
and
SELECT load_file(..)
to do different interpretations of the 
--secure-file-priv option.
     
The patch moves code to the server initialization
routines so that the path always is normalized
once and only once.
      
It was also intended that setting the option
to an empty string should be equal to 
lifting all previously set restrictions. This
is also fixed by this patch.

b89feb5b28 BUG#39053 - UNISTALL PLUGIN does not allow the storage engine
to cleanup open connections

It was possible to UNINSTALL storage engine plugin when binding
between THD object and storage engine is still active (e.g. in
the middle of transaction).

To avoid unclean deactivation (uninstall) of storage engine plugin
in the middle of transaction, additional storage engine plugin
lock is acquired by thd_set_ha_data().

If ha_data is not null and storage engine plugin was not locked
by thd_set_ha_data() in this connection before, storage engine
plugin gets locked.

If ha_data is null and storage engine plugin was locked by
thd_set_ha_data() in this connection before, storage engine
plugin lock gets released.

If handlerton::close_connection() didn't reset ha_data, server does
it immediately after calling handlerton::close_connection().

Note that this is just a framework fix, storage engines must switch
to thd_set_ha_data() from thd_ha_data() if they want to see fit.

be632a2f5a Bug#48449: hang on show create view after upgrading when view contains function of view
SHOW CREATE TABLE on a view (v1) that contains a function whose
statement uses another view (v2), could trigger a infinite loop
if the view referenced within the function causes a warning to
be raised while opening the said view (v2).

The problem was a infinite loop over the stack of internal error
handlers. The problem would be triggered if the stack contained
two or more handlers and the first two handlers didn't handle the
raised condition. In this case, the loop variable would always
point to the second handler in the stack.

The solution is to correct the loop variable assignment so that
the loop is able to iterate over all handlers in the stack.

2a22dc2e01 Bug#49534: multitable IGNORE update with sql_safe_updates
error causes debug assertion

The IGNORE option of the multiple-table UPDATE command was
not intended to suppress errors caused by the
sql_safe_updates mode. This flag will raise an error if the
execution of UPDATE does not use a key for row retrieval,
and should continue do so regardless of the IGNORE option.

However the implementation of IGNORE does not support
exceptions to the rule; it always converts errors to
warnings and cannot be extended. The Internal_error_handler
interface offers the infrastructure to handle individual
errors, making sure that the error raised by
sql_safe_updates is not silenced.

Fixed by implementing an Internal_error_handler and using it
for UPDATE IGNORE commands.

a0bea5eeb1 Bug #48291 : crash with row() operator,select into @var, and
subquery returning multiple rows

Error handling was missing when handling subqueires in WHERE 
and when assigning a SELECT result to a @variable.
This caused crash(es). 

Fixed by adding error handling code to both the WHERE 
condition evaluation and to assignment to an @variable.

fdcf0aff02 A patch and a test case for
Bug#46539 Various crashes on INSERT IGNORE SELECT + SELECT FOR UPDATE.

If a transaction was rolled back inside InnoDB due to a deadlock
or lock wait timeout, and the statement had IGNORE clause,
the server could crash at the end of the statement or on shutdown.

This was caused by the error handling infrastructure's attempt to 
ignore a non-ignorable error.

When a transaction rollback request is raised, switch off 
current_select->no_error flag, so that the following error
won't be ignored.

Instead, we could add !thd->is_fatal_sub_stmt_error to
my_message_sql(), but since in write_record() we switch
off no_error, the same approach is used in 
thd_mark_transaction_to_rollback().

@todo: call thd_mark_transaction_to_rollback() from 
handler::print_error(), then we can easily make sure
that the error reported by print_error is not ignored.

8f6f3dba21 Bug #40877: multi statement execution fails in 5.1.30
Implemented the server infrastructure for the fix:

1. Added a function LEX_STRING *thd_query_string(THD) to return
a LEX_STRING structure instead of char *.
This is the function that must be called in innodb instead of 
thd_query()

2. Did some encapsulation in THD : aggregated thd_query and 
thd_query_length into a LEX_STRING and made accessor and mutator 
methods for easy code updating. 

3. Updated the server code to use the new methods where applicable.

  The Debug Sync Facility allows to place synchronization points
  in the code:

  lock_tables(...)
  
  When activated, a sync point can
  
  - Send a signal and/or
  - Wait for a signal
  
  Nomenclature:
  
  - signal:            A value of a global variable that persists
                       until overwritten by a new signal. The global
                       variable can also be seen as a "signal post"
                       or "flag mast". Then the signal is what is
                       attached to the "signal post" or "flag mast".
  
  - send a signal:     Assign the value (the signal) to the global
                       variable ("set a flag") and broadcast a
                       global condition to wake those waiting for
                       a signal.
  
  - wait for a signal: Loop over waiting for the global condition until
                       the global value matches the wait-for signal.

5e04d4695b BUG#42829: binlogging enabled for all schemas regardless of
binlog-db-db / binlog-ignore-db
      
InnoDB will return an error if statement based replication is used
along with transaction isolation level READ-COMMITTED (or weaker),
even if the statement in question is filtered out according to the
binlog-do-db rules set. In this case, an error should not be printed.
      
This patch addresses this issue by extending the existing check in
external_lock to take into account the filter rules before deciding to
print an error. Furthermore, it also changes decide_logging_format to
take into consideration whether the statement is filtered out from 
binlog before decision is made.

6aea4cebfc Bug #38816: kill + flush tables with read lock + stored
procedures causes crashes!

The problem of that bugreport was mostly fixed by the
patch for bug 38691.
However, attached test case focused on another crash or
valgrind warning problem: SHOW PROCESSLIST query accesses
freed memory of SP instruction that run in a parallel
connection.

Changes of thd->query/thd->query_length in dangerous
places have been guarded with the per-thread
LOCK_thd_data mutex (the THD::LOCK_delete mutex has been
renamed to THD::LOCK_thd_data).

e642140b18 Bug# 30946: mysqldump silently ignores --default-character-set
when used with --tab

1) New syntax: added CHARACTER SET clause to the
  SELECT ... INTO OUTFILE (to complement the same clause in
  LOAD DATA INFILE).
  mysqldump is updated to use this in --tab mode.

2) ESCAPED BY/ENCLOSED BY field parameters are documented as
   accepting CHAR argument, however SELECT .. INTO OUTFILE
   silently ignored rests of multisymbol arguments.
   For the symmetrical behavior with LOAD DATA INFILE the
   server has been modified to fail with the same error:

     ERROR 42000: Field separator argument is not what is
                  expected; check the manual

3) Current LOAD DATA INFILE recognizes field/line separators
   "as is" without converting from client charset to data
   file charset. So, it is supposed, that input file of
   LOAD DATA INFILE consists of data in one charset and
   separators in other charset. For the compatibility with
   that [buggy] behaviour SELECT INTO OUTFILE implementation
   has been saved "as is" too, but the new warning message
   has been added:

     Non-ASCII separator arguments are not fully supported

   This message warns on field/line separators that contain
   non-ASCII symbols.

e1c43705b9 Bug#46265: Can not disable warning about unsafe statements for binary logging
If using statement based replication (SBR), repeatedly calling
statements which are unsafe for SBR will cause a warning message
to be written to the error for each statement. This might lead
to filling up the error log and there is no way to disable this
behavior.

The solution is to only log these message (about statements unsafe
for statement based replication) if the log_warnings option is set.

For example:

SET GLOBAL LOG_WARNINGS = 0;
INSERT INTO t1 VALUES(UUID());
SET GLOBAL LOG_WARNINGS = 1;
INSERT INTO t1 VALUES(UUID());

In this case the message will be printed only once:

[Warning] Statement may not be safe to log in statement format.
          Statement: INSERT INTO t1 VALUES(UUID())

92536e4213 BUG#42851: Spurious "Statement is not safe to log in statement
format." warnings
      
Despite the fact that a statement would be filtered out from binlog, a
warning would still be thrown if it was issued with the LIMIT.
      
This patch addresses this issue by checking the filtering rules before
printing out the warning.

498ac0f598 Bug #34002 uninitialized Rows_examined for some admin queries
such as quit and shutdown

Logging to slow log can produce an undetermined value for
Rows_examined in special cases. In debug mode this manifests
itself as any of the various marker values used to mark
uninitialized memory on various platforms.

If logging happens on a THD object that hasn't performed any
row reads (on this or any previous connections), the
THD::examined_row_count may be uninitialized. This patch adds
initialization for this attribute.

No automated test cases are added, as for this to be
meaningful, we need to ensure that we're using a THD
fulfilling the above conditions. This is hard to do in the
mysql-test-run framework. The patch has been verified
manually, however, by restarting mysqld and running the test
included with the bug report.

a561a95e6c Fix for a valgrind warning due to use of a uninitialized
variable. The problem was that THD::connect_utime could be
used without being initialized when the main thread is used
to handle connections (--thread-handling=no-threads).

a645555432 Bug#44658 Create procedure makes server crash when user does not have ALL privilege
MySQL crashes if a user without proper privileges attempts to create a procedure.

The crash happens because more than one error state is pushed onto the Diagnostic
area. In this particular case the user is denied to implicitly create a new user
account with the implicitly granted privileges ALTER- and EXECUTE ROUTINE.

The new account is needed if the original user account contained a host mask.
A user account with a host mask is a distinct user account in this context.
An alternative would be to first get the most permissive user account which
include the current user connection and then assign privileges to that
account. This behavior change is considered out of scope for this bug patch.

The implicit assignment of privileges when a user creates a stored routine is a
considered to be a feature for user convenience and as such it is not
a critical operation. Any failure to complete this operation is thus considered
non-fatal (an error becomes a warning).

The patch back ports a stack implementation of the internal error handler interface.
This enables the use of multiple error handlers so that it is possible to intercept
and cancel errors thrown by lower layers. This is needed as a error handler already
is used in the call stack emitting the errors which needs to be converted.

0781a30262 Fix for bug#42009: SELECT into variable gives different results to direct SELECT
Problem: storing "SELECT ... INTO @var ..." results in variables we used val_xxx()
methods which returned results of the current row. 
So, in some cases (e.g. SELECT DISTINCT, GROUP BY or HAVING) we got data
from the first row of a new group (where we evaluate a clause) instead of
data from the last row of the previous group.

Fix: use val_xxx_result() counterparts to get proper results.
