Various fixes (cleanups, valgrind, makefiles, ...)

server-tools/instance-manager/Makefile.am:
  increased default_monitoring interval
server-tools/instance-manager/guardian.cc:
  some fixes for proper shutdown

  removed init() prototype, as it was never used
