Method clean up:

Browse Source 
- removed unused methods from MYSQL_STMT handle,
  (left over from PHP's mysqlnd)
- Added execute_generate_request method: This will allow
  Connector/Python to prefill the execute buffer without
  numerous GIL acquire/release calls.
